新疆维吾尔族与汉族男性公务员脂肪肝临床特点及其与颈动脉硬化的关系

摘　　要：目的：探讨新疆维吾尔族与汉族男性公务员脂肪肝临床特点及其与颈动脉硬化的关系。方法选择2011年1月-2012年12月在新疆医科大学第一附属医院进行健康体检的797例维吾尔族、汉族男性公务员，汉族男性405人，维吾尔族男性392人，检测空腹血糖（FBG）、尿酸（UA）、血清总胆固醇（TC）、甘油三酯（TG）、高密度脂蛋白胆固醇（HDL-C）及低密度脂蛋白胆固醇（LDL-C），颈部超声及脂肪肝危险因素进行统计学分析。结果汉族男性公务员405人中脂肪肝检出率为58．23％，维吾尔族男性公务员392人中脂肪肝检出率为73．22％。与汉族男性公务员相比，维吾尔族男性公务员脂肪肝组的体质指数（BMI）、腹围、舒张压（DBP）、UA、FBG、TG、TC水平均最高，HDL最低。维吾尔族与汉族男性公务员颈动脉内膜中层厚度（IMT）差异无统计学意义，但维吾尔族男性脂肪肝组颈部血管斑块检出率明显高于汉族男性脂肪肝组，差异有统计学意义（P ＜0．05）。Logistic回归分析发现族别、BMI、腹围、FBG、TC、颈部斑块为脂肪肝的独立危险因素。维吾尔族男性公务员发生脂肪肝的危险性是汉族男性公务员的1．96倍（95％CI：1．23-3．15）。结论维吾尔族男性公务员脂肪肝检出率高，发生脂肪肝的危险性比汉族男性公务员高，并且更容易发生颈动脉硬化。Objective To analyze the clinical features of fatty liver in male of Uyghur and Han nationality and its relationship with carotid atherosclerosis.Methods 7 9 7 cases,chosen from medical center of The Ffirst Affiliated Hospital of Xinjiang Medical University,have been analyzed biochemical indicators,cervical vessels ultrasound and the risk factors of fatty liver.Results The detection rate of fatty liver was 58.23% in 405 cases of male han nationality and 73.22% in 392 cases of male Uyghur nationality.BMI, diastolic blood pressure （DBP）,UA,FBG,TG,TC levels were the highest in Uyghur men fatty liver group.The difference was statistically significant （P 〈0.05）.Logistic regression analysis found that nationality,BMI,abdominal circumference,FBG,TC,atherosclerotic plaque of cervical vessels were the in-dependent risk factors of fatty liver.The fatty liver risk of Uyghur men was 1.96 times than Han nationality （9 5%CI：1 .2 3-3 .1 5 ）.Conclusion The detection rate of male fatty liver in Uyghur nationality was high,and the risk of fatty liver is higher than Han nationality.
