摘  要:目的观察rhBMP-2/PLA生物活性涂层的骨诱导活性及对种植体周骨组织愈合的影响。方法采用精密车床机械切削纯钛种植体,利用乳化溶剂挥发法制备聚乳酸种植体涂层,通过冷冻干燥法将聚乳酸涂层与rhBMP-2复合,构建rhBMP-2/PLA复合涂层种植体。对9只日本长耳白兔双侧股骨行种植术,每侧制备3个种植孔,随机植入纯钛种植体、rhBMP-2/PLA复合涂层种植体,术后4、8、12周处死动物,对种植体周围骨组织进行大体观察、X线检查、组织学观察及Ⅴ型胶原的免疫组化检测,比较两组种植体周骨组织愈合情况。结果 X线观察显示种植体周骨组织随植入时间延长而密度不断增加,植入后4周、8周rhBMP-2/PLA组种植体周相对骨密度值明显高于纯钛组(P<0.05),差异有统计学意义;12周rhBMP-2/PLA组种植体周相对骨密度值略高于纯钛组(P>0.05),差异无统计学意义。组织学观察:镜下4周rhBMP-2/PLA组可见种植体界面处大量局灶性编织骨形成,新骨边缘衬有一层排列紧密的成骨细胞,成骨活跃;纯钛组种植体界面处均可见少量编织骨,未见成骨细胞活跃增殖。8周rhBMP-2/PLA组种植体界面处编织骨数量明显增加,钙化程度有所增加,且出现部分板层骨;纯钛组种植体界面处仍为局灶性编织骨,且钙化度较低,板层骨不明显。12周rhBMP-2/PLA组种植体界面处板层骨数量继续增加,骨髓腔不断扩大,且新骨融合成蜿蜒迂曲的骨小梁,正在形成哈佛氏系统。纯钛组种植体界面处也出现部分板层骨及骨小梁结构。结论rhBMP-2/PLA复合涂层具有良好的骨诱导活性,能促进种植体周骨组织的早期愈合。Objective To observe the osteoinductivity of the recombinant human bone morphogenetic protein-2/polylactic acid (rhBMP-2/PLA) composite coating and its effect on peri-implant bone healing. Methods Pure titanium implants were manufactured by precision lathe cutting machine, and PLA coatings were prepared with emulsion solvent evaporation, rhBMP-2/PLA-coated implants were constructed by freeze-drying technique. Nine Japanese rabbits were involved in the study. Three implant holes were prepared in each femur, and rhBMP-2/PLA coated implants and pure titanium implants were implanted randomly. 4, 8, 12 weeks after implantation, animals were killed and all specimens were studied by X-ray, histological observation and immunohistochemical analysis of collagen type V to compare the difference in the bone healing condition around implants between the two groups. Results X-ray tests indicated that in 4~, 8~, 12~ week, peri-implant bone density continuously increased in two groups. In 4~ and 8~ week, the density in the rhBMP-2/PLA group was significantly higher than that in the pure titanium implant group (P〈0.05). And in 12th week, the density in the rhBMP-2/PLA group was a little higher than that in the pure titanium implant group (P〉0.05). Histological observations revealed woven bone formation and a layer of tightly packed osteoblasts in the interface between implant and bone in rhBMP-2/PLA group in 4th week, whereas only small amount of woven bone and proliferation of osteoblasts could be seen in the pure titanium group. In 8th weeks, the formation of woven bone and degree of calcification increased and lamellar bone appeared in the rhBMP-2/PLA group. And in the pure titanium group, there only existed focal woven bone and low degree of calcification. In 12th week, lamellar bone continued increasing and bone marrow cavity expanded and Haversian system were formed. In the pure titanium group, lamellar bone and trabecular structure also appeared.
