摘 要:通过检测HR-HPV感染的宫颈脱落细胞和宫颈活检中hTERC的表达,并对实验结果进行比较分析,探讨其与宫颈病变的相关性。对84例HR-HPV阳性(宫颈活检结果为正常及慢性炎12例、CINⅠ20例、CINⅡ20例、CINⅢ20例、SCC 12例)应用荧光原位杂交技术(FISH检测),分别检测患者的宫颈脱落细胞和宫颈活检组织中hTERC的表达;应用受试者工作特征曲线(ROC曲线),分别确立细胞学和组织学中宫颈高度鳞状上皮内病变的最佳界值。实验结果为:①在正常及慢性炎、CINⅠ、CINⅡ、CINⅢ、SCC组所对应的宫颈脱落细胞中,hTERC基因扩增的阳性率分别为8.33%(1/12)、15.00%(3/20)、80.00%(16/20)、90.00%(18/20)、100%(12/12),与宫颈病变的恶性程度呈正相关(rs=0.713,p<0.001)。②在正常及慢性炎、CINⅠ、CINⅡ、CINⅢ、SCC组所对应的的宫颈活检中,hTERC基因扩增的阳性率分别为16.67%(2/12)、25.00%(5/20)、80.00%(16/20)、95.00%(19/20)、100%(12/12),与宫颈病变的恶性程度呈正相关(rs=0.671,p<0.001)。③宫颈脱落细胞中hTERC的扩增在预测≥CINⅡ病变中的敏感度、特异度,分别为92.00%和82.35%;宫颈活检组织中hTERC预测≥CINⅡ的敏感度、特异度,分别为87.03%和83.33%.实验可以得出:hTERC在HR-HPV阳性宫颈病变患者宫颈脱落细胞和宫颈活检组织中的表达具有一致性,hTERC在预测HR-HPV阳性且宫颈细胞学检查为异常的宫颈病变的病理结果中具有重要的意义。By examining the HR - cervical exfoliated cells of HPV infection and cervical biopsy in hTERC expression, and comparing with the result of the experiment analysis, explore its correlation with cervical lesions. In 84 cases of HR - HPV positive ( cervical biopsy results as normal and 12 cases of chronic inflammation, CIN Ⅰ 20 cases, CIN Ⅱ 20 cases, CIN Ⅲ 20 cases, 12 cases SCC )application of fluorescence in situ hybridization ( FISH ) technology, respectively in patients with cervical exfoliated cells and cervical biopsy tissues hTERC expression; Applied the receiver-operating characteristic curve ( ROC curve ) , respectively, to establish cytological and histological height of cervical squamous intraepithelial lesions in the best value. The results as follows: ( 1 ) in normal and chronic inflammation, CIN Ⅰ, CIN Ⅱ, CIN Ⅲ, scc group of cervical exfoliated cells, hTERC gene amplification of positive rate was 8.33% ( 1/12 ), 15.00% ( 3/20 ), 80.00%( 16/20 ), 90.00% ( 18/20 ) , 100% ( 12/12 ) , and the malignant degree of cervical lesions were positively correlated ( rs = 0.713, p 〈 0.001 ) . ( 2 ) in normal and chronic inflammation, CIN Ⅰ, CIN Ⅱ, CIN Ⅲ, SCC of the set of cervical biopsy, hTERC gene amplification of positive rate was 16.67% ( 2/12 ) , 25.00% ( 5/20 ) , 80.00% ( 16/20 ) , 95.00% ( 19/20 ) , 100% ( 12/12 ) , and the malignant degree of cervical lesions were positively correlated( rs = 0.671, p 〈 0.001 ).( 3 )in cervical exfoliated cells hTERC amplification in the prediction of CIN or Ⅱ lesions in sensitivity, specific degrees, 92.00% and 82.35%, respectively; Cervical biopsy tissues hTERC predict acuity CIN Ⅱ sensitivity, specific degrees, 87.03% and 83.33%, respectively. The experiments can be concluded that hTERC in HR - HPV positive patients with cervical lesion cervical exfoliated cells and cervical biopsy samples of expression consistent, hTERC in predicting HR - HPV positive and abnormal cervical
