进展期胃癌患者术中使用氟尿嘧啶植入剂临床观察

摘　　要：目的 探讨进展期胃癌患者术中使用氟尿嘧啶植入剂对术后不良事件的发生情况及预后的影响.方法 回顾性分析天津医科大学肿瘤医院2007年1月至2009年1月接受胃癌根治术治疗的124例进展期胃癌患者的临床资料,术中植入氟尿嘧啶植入剂患者为观察组（64例）,术中未进行氟尿嘧啶植入剂治疗者为对照组（60例）.记录术中接受氟尿嘧啶植入剂区域性化疗患者的术后15 d内体温、腹腔引流液和不良反应事件的发生率以及生存情况,并与对照组进行比较分析.结果 两组患者一般资料比较,差异无统计学意义（P＞0.05）.两组患者术后均无全身性不良事件记录.腹腔局部不良事件中,观察组腹胀10例（15.6％）,对照组9例（15.0％）,均为轻度腹胀;两组差异无统计学意义（P＞0.05）.两组患者切口愈合情况均未见异常.观察组3年生存率为64.3％,高于对照组的42.4％,差异有统计学意义（P=0.018）.结论 进展期胃癌患者术中置入氟尿嘧啶植入剂是可以耐受的,对提高进展期胃癌患者术后的生存率有积极作用.Objective To investigate the postoperative adverse events and survival of patients with sustained-released fluorouracil implanted during operation.Methods Data of 124 patients with advanced gastric cancer undergoing radical operation in Tianjin Medical University Cancer Institute and Hospital from January 2007 to January 2009 were analyzed retrospectively.All the patients were divided into two groups according to whether intra-operative fluorouracil was implanted or not.The treatment group （n=64） was implanted with fluorouracil in abdominal cavity after radical resection.The control group （n=60） did not receive fluorouracil implant in abdominal cavity after radical resection.Abdominal drainage fluid,temperature and adverse events within 15 postoperative days and 3-year survival were observed and compared between the two groups.Results Pathological findings of the two groups were similar.No statistical significances existed in abdominal drainage fluid,temperature and adverse events within 15 postoperative days （P〉0.05）.The 3-year survival rate was higher in treatment group（64.3% vs.42.4%,P=0.018）.Conclusion Intraoperative sustainedreleased fluorouracil implants are safe and tolerable,and can improve the survival rate of patients with advanced gastric cancer.
