超声乳化白内障吸除后房型人工晶状体植入术治疗闭角型青光眼合并白内障的疗效

摘　　要：目的观察超声乳化白内障吸除后房型人工晶状体植入术治疗闭角型青光眼合并白内障的疗效。方法选择原发性闭角型青光眼合并白内障患者56例(60眼),分为观察组和对照组。观察组32眼采用超声乳化白内障吸除后房型人工晶状体植入术;对照组28眼采用超声乳化白内障吸除+人工晶状体植入联合小梁切除术。对2组患者手术前后的视力、眼压、前房深度及术后并发症进行比较。结果与术前比较,术后2组患者视力均显著提高,眼压显著降低(P<0.05);2组患者术后视力和眼压比较差异无统计学意义(P>0.05)。与对照组比较,观察组患者术后炎症反应少(P<0.05),并发症发生率低(P<0.05)。结论对闭角型青光眼合并白内障患者,超声乳化白内障吸除后房型人工晶状体植入术是一种有效方法,且术后炎症反应少,并发症发生率低。Objective To observe the clinical effect of phacoemulsification with posterior chamber intraocular lens implantation on the treatment of angle-closure glaucoma. Methods A total of 56 patients with primary angle-closure glaucoma combined cataract（ 60 eyes） were chosen and divided into observation group and control group. Observation group（ 32 eyes）were treated with phacoemulsification and intraocular lens implantation while control group（ 28 eyes） were treated with phacoemulsification and intraocular lens implantation combined trabeculectomy. The visual acuity,intraocular pressure,anterior chamber depth and the postoperative complications of the two groups were compared before and after operation. Results Compared with preoperative index,the postoperative visual acuity in the two groups improved significantly,and the postoperative intraocular pressure significantly reduced（ P〈0. 05）. No statistically significant difference was found in the two groups of postoperative vision and intraocular pressure（ P〉0. 05）. Compared with the control group,the observation group had less postoperative inflammatory response（ P〈0. 05） and lower complication rate（ P〈0. 05）. Conclusion For patients with angle-closure glaucoma combined cataract,it is an effective treatment of phacoemulsification with posterior chamber intraocular lens implantation,which has less postoperative inflammatory response and lower complication rate.
