聚明胶肽联合尼莫地平防治破裂动脉瘤栓塞术后脑血管痉挛的临床研究

摘　　要：目的：总结应用聚明胶肽联合尼莫地平防治破裂动脉瘤栓塞术后脑血管痉挛（CVS）的疗效。方法将破裂动脉瘤栓塞术后68例患者分为两组，研究组36例给予聚明胶肽联合尼莫地平，对照组32例给予尼莫地平，观察两组CVS和脑梗死的发生情况及出院时GOS评分，对比两组疗效。结果研究组出现无症状性CVS 11例（30．56％）、症状性CVS 1例（2．78％）、无脑梗死发生，对照组出现无症状性CVS 12例（37．50％）、症状性CVS 3例（9．38％）、脑梗死2例（6．25％），两组比较差异有统计学意义（P＜0．05）；研究组的 GOS 评分明显优于对照组（P＜0．05）。结论联合应用聚明胶肽尼莫地平是防治破裂动脉瘤栓塞术后CVS的安全有效的方法，其疗效优于单独使用尼莫地平。Objective To investigate and summarize the efficacy of polygeline combined with nimodipine in preventing of cerebral vasospasm（CVS）after embolization of ruptured aneurysms.Methods Sixty-eight cases of embolization of ruptured aneurysms were divided into two groups.In the study group, 36 patients were given polygeline combined with nimodipine,while 32 patients were given nimodipine in the control group.The occurrence of CVS and cerebral infarction and GOS score at discharge were observed.The clinical effects of the two groups were compared.Results In the study group,asymptomatic CVS occurred in 1 1 patients（30.56%）,symptomatic CVS occurred in 1 patient（2.78%）and no cerebra infarction occurred.While in the control group,asymptomatic CVS occurred in 12 patients（37.50%）, symptomatic CVS occurred in 3 patients（9.38%）and cerebral infarction occurred in 2 patients（6.25%）. The difference was statistically significant between the two groups （P〈0.05 ）.GOS score in 〈the study group was significantly higher than the control group（P〈0.05 ）.Conclusion Polygeline combined with nimodipine is a safe and effective method in preventing cerebral vasospasm （CVS ）after embolization of ruptured aneurysms,which is more effective than nimodipine alone.
