与抗精神病药物相关的浆细胞性乳腺炎10例病例研究

摘　　要：目的探究口服抗精神病药物与浆细胞性乳腺炎二者的关系,口服药物是通过何种途径加重或诱发浆细胞性乳腺炎。方法对北京中医药大学东方医院乳腺科接诊的10例口服抗精神病药物后并发浆细胞性乳腺炎患者进行分析总结。结果 10例口服抗精神病药物患者所患乳腺疾病经最后病理证实均为浆细胞性乳腺炎。其病变均具有病灶范围较大;脓腔为单腔,脓液均为浓稠的淡黄色油脂与乳汁的混合液体;乳腺腺体内导管广泛扩张;双侧发病率较高的特点。且手术治疗前查泌乳素水平平均值为1 150.64 uIU·mL-1,远高于正常范围。结论口服抗精神病药物会导致血清泌乳素水平升高,这可能是诱发和加重浆细胞性乳腺炎的一个因素。另外,口服抗精神病药物引发浆细胞性乳腺炎或许还有其他我们未知的作用机制,对此还有待进一步的研究。Objective Toexploretherelationshipbetweenoraladministrationofantipsychoticsand plasmacellmastitisandthathowthedrugaggravatedorinducedplasmacellmastitis.Methods Tencases of plasma cell mastitis induced by antipsychotics were analyzed in Breast Department of Dongfang Hospital, BeijingUniversityofTraditionalChineseMedicine.Results Itwasconfirmedpathologicallythat10cases were plasma cell mastitis after oral administration of antipsychotics,characterized as large lesion,single pus cavity,mixed liquid of thick yellow fat and milk,extensive expansion of tube in mammary gland and high inci-dence of bilateral sides.The mean of prolactin was 1 150.64 uIU·mL-1 before surgery,quite higher than the normalrange.Conclusion Theoraladministrationofantipsychoticsincreasesprolactin,whichisprobably the factor for inducing and aggravating plasma cell mastitis.Additionally,antipsychotics associated plasma cell mastitis maybe have some other unknown effect mechanism and a further study is expected.
