中西医结合改善痉挛型脑瘫双下肢运动功能临床研究

摘　　要：目的探索治疗痉挛型脑瘫有效的中西医临床治疗方法。方法将120例患儿随机分为治疗组与对照组,各60例。对照组在腰骶段脊神经后根选择性部分切断术(SPR)后给予常规康复训练,治疗组在对照组的基础上给予双下肢局部熏蒸及药浴治疗;评价治疗前后两组患儿双下肢粗大运动功能评分及前后差值,比较两组总体疗效。结果治疗组总有效率为96.67%,对照组总有效率为86.67%,两组比较,差异有统计学意义(P<0.05);患儿粗大运动功能评定,两组自身前后比较,差异均有统计学意义(P<0.05,P<0.01)),治疗后组间比较,治疗组优于对照组(P<0.05),两组评分前后差值比较,治疗组优于对照组(P<0.01)。结论腰骶部SPR术结合中药局部熏蒸及药浴能有效降低痉挛型脑瘫患儿的肌张力,改善其运动功能,值得临床推广。Objective TodiscusstheclinicaleffectivetherapeuticmethodofintegratedChineseand westernmedicineforspasticcerebralpalsy.Methods Onehundredandtwentychildrenwererandomizedin-to a treatment group and a control group,60 cases in each one.In the control group,the conventional rehabili-tation was applied after the selective posterior rhizotomy（SPR）.In the treatment group,on the basic treatment as the control group,the local herbal steaming bathing therapy was given in the lower limbs.The gross motor function and the different value before and after treatment were evaluated in the two groups.The overall effi-cacywascomparedbetweenthetwogroups.Results Thetotaleffectiveratewas96.67%inthetreatment group and was 86.67% in the control group,indicating the significant difference in comparison（P〈0.05）. In the self-comparison,the difference in the gross motor function evaluation was significant（P〈0.05,P〈0.01）;after treatment,in comparison between the two groups,the value in the treatment was better than that in the control group（P〈0.05）.In comparison of the value difference before and after evaluation,the result inthetreatmentgroupwasbetterthanthatinthecontrolgroup（P〈0.01）.Conclusion Thelumbosacral SPR combined with local herbal steaming and bathing therapy effectively reduces muscle tone and improves motor function in the patients of spastic cerebral palsy and deserves to be promoted in clinic.
