复方丹参滴丸对比羟苯磺酸钙治疗非增生期糖尿病视网膜病变的系统评价  

Compound Danshen dripping pills versus calcium dobesilate therapy in patients with nonproliferative diabetic retinopathy： a systematic review

摘　　要：目的系统评价复方丹参滴丸（compound Danshen dripping Pills，CDDP）治疗非增生期糖尿病视网膜病变（Non-proliferative diabetic retinopathy，NPDR）患者的有效性。方法计算机检索Pubmed、OVID、EMbase、Cochrane Library、IsI、CBM、CNKI、VIP、万方等数据库；手工检索相关杂志纸质版。纳入所有CDDP与羟苯磺酸钙治疗NPDR的随机对照试验（Randomized controlled trial，RCT）。根据Cochrane系统评价方法，由两位评价员按照纳入排除标准独立进行质量评价和资料提取，核对后采用RevMan5．2软件进行Meta分析。结果共纳入6个随机对照试验。Meta分析结果显示CDDP治疗对NPDR患者总有效性优于羟苯磺酸钙[RR=1．54，95％CI（1．03，2．32），P=0．04]。CDDP治疗对微血管瘤、出血有效性、荧光渗漏的效果较羟苯磺酸钙好。结论CDDP每日3次，每次15粒较羟苯磺酸钙每日3次，每次500mg治疗NPDR疗效更好，但仍需更高质量的长期随访研究进行系统评价予以验讧。Objective To evaluate the efficacy of compound Danshen dripping pills （CDDP） treatment in Nonproliferative diabetic retinopathy （NPDR）. Methods Searched PubMed, OVID, EMbase, Cochrane Library, IsI, CBM, CNKI, VIP, Wan fang through the internet. We also handsearched print edition of related journals. All Randomized controlled trials（RCTs） of CDDP versus calcium dobesilate to NPDR would be included in the study. According to the method of Cochrane systematic review, the selection of studies, extraction of data and assessment of methodological quality were performed independently by two reviewers. Meta-analyses were performed using Review Manager 5.2 software. Results Six RCTs were included. The results of meta-analyses showed that total efficiency of CDDP treatment to NPDR was superior to calcium dobesilate[RR=1.54,95%CI（1.03,2.32）,P=0.04].Microaneurysms, bleeding efficient,effectiveness of fluorescein leakage of CDDP was superior to calcium dobesilate. Conclusion CDDP 15 pills a time, three times a day seems have stronger efficacy than calcium dobesilate 500mg a time, three times a day, but longer follow-up and higher quality research will be needed to vertify it.
