重建内外括约肌原位肛门再造术治疗20例中低位直肠癌

摘　　要：目的 :观察重建内外括约肌原位肛门再造术治疗中低位直肠癌的临床效果。方法 :对 2 0例中低位直肠癌设计一种再造肛门内外括约肌术式 ,即手术操作按miles术进行。结肠折叠再造内括约肌 ,保留耻骨直肠肌两断端 ,借助会阴浅横肌再造外括约肌。结果 :全组病例无手术死亡 ,术后功能效果较好 19例 ,效果不佳 1例。结论 :重建内外括约肌原位肛门再造术临床效果较好 ,值得推广。Objective:To investigate the clinical effect of restoration of anus in situ by internal and external sphincter reconstruction in the treatment of middle and low rectal carcinoma.Methods:An operation was designed to reconstruct internal and external sphincter of anus in 21 patients with middle and low rectal carcinoma. The operation was used by Mile's procedure.Internal sphincter was reconstruced by folding colon and keeping two amputated end of puborectal muscle to reconstruct external sphincter by perineal superficial transverse muscle.Results:In all 21 patients there is no death in operation. 14 cases have an excellent postoperative function,4 cases have a good postoperative function, only one has common postoperative function.Conclusion:The reconstruction of anus in situ by internal and external sphincter reconstruction has a good clinical effect and is worthy of popularization.
