非痴呆型血管性认知障碍的神经心理学研究

摘　　要：目的探讨测试非痴呆型血管性认知障碍者认知功能的新方法及其使用价值。方法应用YWG神经心理训练系统和修订版连线测验（TMT）对非痴呆型血管性认知障碍（VCIND）组（60例）、正常对照组（60例）进行测验。对比观察两组单侧脑功能、图形记忆、汉字记忆和数字连线测验结果，以及各指标与TMT得分的相关性。结果和对照组相比，VCIND 组YWG各项目反应时延长27%~158%（ P ﹤0.01），错误率增高27%~63%，连线测验两部分得分也明显降低（ P ﹤0.01）。VCIND组内，男女患者之间、脑出血和脑梗死患者之间各项目得分无显著差异（ P ﹥0.05）。实验组和对照组单侧脑功能反应时均与TMT得分有明显相关性（P﹤0.01）。结论 YWG神经心理训练系统测试结果可反映VCIND执行功能损害，可作为VCIND患者的认知功能测试项目之一用于临床。Objective This paper is explored to evaluate the applicability of a new neuropsychological system for testing cognitive dysfunction in patients with vascular cognitive impairment without dementia（VCIND）. Methods Sixty normal elderly persons and 60 patients with VCIND were evaluated by two kinds of neuropsychological tests：YWG neuropsychological training system and trail making test（TMT）- Chinese version. The scores of TMT and three YWG neuropsychological training system tests - unilateral cerebral function test,graph memory test and Chinese character memory test were compared between these two groups. The correlation of mean response time in three YWG neuropsychological training system tests and TMT scores had been analyzed. Results Patients in VCIND group had a longer mean response time by 27% ~158%（ P ﹤ 0. 01）and a higher error rate by 27% ~63%（ P ﹤0. 01）. Scores of TMT in VCIND group were significantly lower than those of control group（P﹤0. 01）.In VCIND group,there was no significant difference in overall means response time and error rate between males and females or between patients with cerebral infarction or cerebral hemorrhage. The mean response time of unilateral cerebral function,graph memory and Chinese character memory were all related to scores of TMT（0. 322≤r≤0. 883,P﹤0. 01）. Conclusion The result from the test by YWG neuropsychological training system indicate that it can demonstrate the executive dysfunction of VCIND;hence these tests can be clinically applied.
