非小细胞肺癌脑转移瘤的立体定向放射治疗

摘　　要：目的 探讨立体定向放射治疗（SRT）加或不加全脑放疗（WBRT）对非小细胞肺癌（NSCLC）有限数目脑转移瘤治疗的疗效.方法 回顾性分析NSCLC脑转移瘤（1～4枚）患者75例,其中行SRT患者35例,行SRT ＋WBRT患者40例.分析两组的治疗有效率、总生存期及放射治疗后的副作用情况.结果 SRT组与WBRT ＋SRT组的总有效率分别为91.5％和87.5％,差异无统计学意义（P＞0.05）;全组患者的中位总生存期为14.7个月,其中SRT组为13.7个月,SRT ＋WBRT组为15.4个月,差异无统计学意义（P=0.180）;两种治疗方式均未见严重不良反应.结论 SRT是NSCLC有限数目脑转移瘤患者安全有效的治疗方式,单纯SRT可取得与SRT ＋WBRT相似的疗效.Objective To analyze the effect of stereotactic radiotherapy（SRT） with or without whole brain radiotherapy（WBRT） in the treatment of hmited number brain metastases of non-small cell lung cancer （NSGLC）.Methods 75 NSCLC patients with brain metastases within 4 lesions were retrospectively analyzed.35cases were treated with SRT alone and 40 cases were treated with SRT ＋ WBRT.The therapeutic effects,overall survival and radiotherapeufic side effects of the two groups were compared.Results The total effective rate of SRT group and WBRT ＋ SRT group were 91.5% and 87.5% respectively.There was no significant difference between the two groups （P 〉0.05）.For all the patients,the median overall survival time was 14.7 months.There was no difference between SRT alone group and SRT ＋ WBRT group （13.7 months vs 15.4 months,P =0.180）.There was no serious side effectin in two groups.Conclusions Stereotactic radiotherapy was an effective and safe treatment for limited number brain metastases of NSCLC.The patients who accepted SRT alone or SRT ＋ WBRT had the similar overall survival time.
