化放疗对宫颈癌组织SP细胞和非SP细胞影响的初步研究

摘  要:目的 探讨化放疗前后宫颈癌组织中SP细胞和非SP的水平及其生物学特性的变化.方法 选取确诊宫颈癌患者60例(IB2-IIIB期),30例给予静脉化疗2程PTX+DDP,PTX 175 mg/m2,静脉注射,第1天;DDP 80 mg/m2,静脉注射,第1~3天;21d为1周期.化疗组患者在化疗前和2程化疗后各取材1次.30例患者行根治性放疗,放疗患者在放疗前和放疗2周后(即大野照射DT20 Gy/10次)各取材1次.采用流式细胞技术检测和比较治疗前后宫颈癌组织原代培养细胞中SP细胞和非SP细胞的水平及其生物学标记的变化.结果 宫颈癌组织SP细胞水平在治疗后比例明显增加[化疗前后(1.63±0.50)%vs(7.96±1.39)%]; [放疗前后(1.63±0.46) %vs (4.51±0.72)%],差异有统计学意义(P<0.05).SP细胞生物学标记(ABCG2、CD133、CD43、P63)的表达率治疗前后无统计学差异,而Ki-67和MDR在治疗后比例明显增加[Ki-67化疗前后(14.77± 1.97) %vs (23.49±2.23)%], [放疗前后(14.61±1.40%) vs (23.10±1.82)%]; [MDR化疗前后(59.31±11.13) %vs (68.91±9.94)%], [放疗前后57.42±11.22% vs 59.37±10.73)%],差异有统计学意义(P<0.05).SP细胞周期和细胞增殖状况在治疗前后无明显变化,差异无统计学意义(P>0.05).非SP细胞生物学标记Ki-67和MDR在治疗后比例明显增加[Ki-67化疗前后(40.48±3.35)%vs (46.29±3.71)%], [放疗前后(40.22±4.70)%vs (45.82±4.55)%]; [MDR化疗前后(17.76±1.61)%vs (34.36±3.15)%], [放疗前后(17.66±1.71)%vs (19.21±1.98)%],差异有统计学意义(P<0.05).治疗后的非SP细胞在G0/1期和S期的比例明显下降,在G2/M期、凋亡和坏死比例明显增加,差异均有统计学差异(P<0.05).结论 化放疗主要杀伤非SP细胞,化放疗后的SP细胞保持肿瘤干细胞生物学特性,对化放疗具有抗拒性,这为进一步研究宫颈癌干细胞的靶向治疗奠定基础.Objective To compare the proportion and biological characteristics of side population cells and non side population cells before and after chemoradiotherapy.Methods We selected 60 cases diagnosed as cervical cancer and staged IB2-IIIB,30 cases were given paclitaxel at a dose of 175 mg/m2 combined with cisplatin at a dose of 80 mg/m2,the chemotherapy was repeated every 21 days.The human cervical cancer cells were obtained from fresh human cervical cancer tissue before chemotherapy and after two chemotherapy processes respectively.Another 30 cases received radiotherapy,the human cervical cancer cells were obtained from fresh human cervical cancer tissue before radiotherapy and two weeks after radiotherapy (wide field radiation DT20Gy/10F) respectively.Flow cytometry was used to isolate and compare the proportion and biological characteristics between side population cells and non side population cells from fresh human cervical cancer tissue before and after chemoradiotherapy.Results The proportion of SP cells had statistically significant difference between before and after treatment (before and after chemotherapy 1.63 ± 0.50% vs 7.96 ± 1.39%,before and after radiotherapy 1.63 ± 0.46% vs 4.51 ± 0.72%).The proportions of ABCG2,CD133,CD43 and P63 of SP cells had no statistically significant difference between before and after treatment,but the proportions of Ki-67 and MDR of SP cells were significantly higher than pretherapy and statistically significant difference (Ki-67 before and after chemotherapy 14.77 ± 1.97% vs 23.49 ± 2.23%,before and after radiotherapy 14.61 ± 1.40 vs 23.10 ± 1.82%; MDR before and after chemotherapy 59.31 ± 11.13% vs 68.91 ± 9.94%,before and after radiotherapy 57.42 ± 11.22% vs 59.37 ± 10.73%).There were no statistically significant differences in cell cycle or cell apoptosis of SP cells.The proportions of Ki-67 and MDR of non SP cells were significantly higher than pre-therapy and had statistically significant difference (Ki-67 before and afte
