摘 要:目的研究乳腺癌患者外周血中Th17细胞、Treg细胞及相关细胞因子的表达水平,探讨其表达与淋巴结转移的关系.方法运用流式细胞分析技术(FACS)检测35例术前乳腺癌患者Th17细胞、Treg细胞的比例,采用ELISA法检测相关细胞因子IL-23、IL-10、IL-35、TGF-β的表达水平.结果乳腺癌外周血中Th17细胞和Treg细胞的比例均高于健康对照组(P<0.05),淋巴结转移组Th17细胞和Treg细胞比例均高于无淋巴结转移组(P<0.05),且Treg细胞比Th17细胞升高的更为明显,淋巴结转移患者的Th17/Treg的比值低于无淋巴结转移者(P<0.05),相关细胞因子IL-23、IL-10、TGF-β水平较健康对照组升高,IL-35水平下降,但IL-23在乳腺癌淋巴结转移和非淋巴结转移组无明显差异(P>0.05),而IL-10、TGF-β、IL-35水平在淋巴结转移组和无淋巴结转移组存在差异(P<0.05).结论细胞因子IL-23、IL-10、TGF-β和Th17细胞和Treg细胞的变化趋势相一致,IL-35和Treg细胞的升高呈负相关,说明细胞因子参与了肿瘤微环境的调节,Thl7/Treg平衡失调或许是导致肿瘤进展的因素之一.Objective To investigate expression of the Th17 cells and Treg cells in breast cancer tumor and related cytokines in the peripheral blood,and study its relationship with lymphatic metastasis.Methods Flow cytometry was applied to test Th17 cells and Treg cells in the peripheral blood of both 35 cases of breast cancer and 20 controls.ELASA method was used to test the expression cytokines of IL-23,IL-10,IL-35 and TGF-β.Results The frequency of Th17 cells and Treg cells in breast cancer was significantly higher that in healthy volunteers (P <0.05).The frequency of Th17 cells and Treg cells in patients of lymphatic metastasis was significantly higher that in patients of non-lymphatic metastasis (P<0.05),then Treg cells increased more obviously than Th17 cells.The radio of Th17/Treg cells in patients of lymphatic metastasis was significantly lower than in patients of non-lymphatic metastasis (P<0.05).IL-23,IL-10 and TGF-β expression levels were higher than those in healthy control group (P<0.05),but IL-35 expression levels were lower.There was no significant difference in IL-23 expression between lymphatic metastasis group and non-lymphatic metastasis group (P>0.05),while there were significant differences in expression of IL-10,TGF-β and IL-35 between two groups (P< 0.05).Conclusions The trend of changes in the related cytokines IL-23,IL-1O and TGF-β is in agreement with that of Th17 cells and Treg cells in the peripheral blood of breast cancer.The rise of IL-35 and Treg cells shows a negative correlation.The imbalance of Th17/Treg may be one of the factors leading to tumour progression.
