摘 要:目的探讨低场MR对子宫旁实性肿瘤的诊断价值。方法回顾性分析经手术病理证实的25例子宫旁实性肿瘤患者的临床及MR影像资料。结果 25例子宫旁实性肿块者,12例卵巢性索间质肿瘤(OSCST),其中卵泡膜细胞瘤4例、纤维瘤3例、颗粒细胞瘤3例、硬化性间质瘤1例、支持-间质细胞瘤1例;6例T2WI呈低信号,3例卵泡膜细胞瘤轻度强化、2例纤维瘤不强化;6例T2WI呈高信号,明显强化,包括3例颗粒细胞瘤、1例卵泡膜细胞瘤、1例支持-间质细胞瘤及1例硬化性间质瘤。另13例阔韧带平滑肌瘤(BLL),在T2WI上9例呈低信号、3例呈高信号、1例呈等信号;9例增强扫描者7例(T2WI上低信号3例、等或高信号4例)明显强化、2例(T2WI上呈低信号)轻度强化。本组正确诊断21例(84%),误诊为BLL 2例、OSCST 1例、胃肠间质瘤1例。结论 OSCST、BLL在低场MRI上表现具有一定特征性,对该类肿块具有一定的诊断价值。Objective To assess the clinical value of low-field MRI scanning in diagnosing parauterine solid masses. Methods A total of 25 cases with surgically and pathologically proved parauterine solid tumors were enrolled in this study. The clinical data and MR imaging materials were retrospectively analyzed. MR imaging features were correlated with pathological resuhs. Results Of the 25 cases with parauterine solid masses, ovary sex cord stromal tumor (OSCST) was seen in 12 and broad ligament leiomyoma (BLL) in 13. The OSCST included ovarian thecoma (n =4), fibroma (n =3), granular cell tumor ( n = 3 ), sclerosing stromal tumor (n = 1 ) and Sertoli-Leydig cell tumor ( n = 1 ). In six cases the lesions were manifested as low signal on T2WI with obvious enhancement, which included granular cell tumor (n = 3), ovarian thecoma ( n = 1 ), Sertoli-Leydig cell tumor ( n = 1 ) and sclerosing stromal tumor ( n = 1 ). Of the 13 patients with BLL, low signal on T2WI was seen in 9, high signal in 3 and iso-intensity signal in one. Contrast-enhanced MRI scanning was carried out in 9 patients. The lesions showed remarkable enhancement in 7 patients and were characterized by low-intensity signal on T2 WI (n = 3 ) or by iso- or high-intensity signal (n = 4 ). In other two patients the lesions were manifested as low- intensity signal on T2WI with slight enhancement. In this series, correct diagnosis was made in 21 cases (84%) and in 4 patients the lesions were misinterpreted as BLL ( n = 2), as OSCST ( n = 1 ), or as gastrointestinal stromal tumor ( n = 1 ). Conclusion Both ovary sex cord stromal tumor and broad ligament leiomyoma have certain characteristic manifestations on low-field MRI scanning, which is very useful for making a correct diagnosis.
