男方染色体平衡易位不育夫妇行体外受精-胚胎移植术后的临床妊娠结局

摘　　要：目的初步探讨男性染色体平衡易位携带者行常规IVF或ICSI技术治疗后的临床妊娠结局,为此类患者的辅助生育治疗及遗传咨询提供指导。方法比较研究36例男方携带染色体平衡易位的不育症夫妇(观察组)及751例染色体正常的不育症夫妇(对照组)在行常规IVF或ICSI治疗后的临床妊娠率、胚胎着床率、多胎妊娠率、活胎分娩率、流产率等指标。结果 36例男方染色体平衡易位的不育症夫妇,共进行38次取卵,完成38个新鲜胚胎移植治疗周期和13个冷冻胚胎移植治疗周期,其中有24个周期获得临床妊娠,5个周期早期自然流产,总计18对夫妇成功分娩了24个婴儿,每取卵周期活胎分娩率达47.4%(18/38)。随访未见晚期流产、死胎、畸胎或出生畸形儿的病例。观察组与对照组间临床妊娠率、胚胎着床率、多胎妊娠率、活胎分娩率、流产率均无显著性差异(P>0.05)。结论对于男方携带染色体平衡易位的不孕不育患者,除PGD外,常规的IVF或ICSI也是一种可供考虑的选择,这类患者行常规IVF或ICSI可获得与非携带者相似的妊娠结局。Objective：To investigate the pregnancy outcomes of couples with paternal balanced translocations after conventional in vitro fertilization（IVF）or intracytoplasmic sperm injection（ICSI）in order to provide clinical data and genetic counseling for balanced translocation carriers.Methods：Thirty-six couples with paternal balanced transloeation including 23 reciprocal translocations and 13 Robertsonian translocations and 751 chromosomally normal couples were involved in the study.The pregnancy rate,implantation rate,multiple pregnancy rate,miscarriage rate and live birth rate in the fresh or frozen embryo transfer（ET）cycles were compared between the two groups.Results：In 36 translocation couples,38 ovum pickup cycles were performed,and 38 fresh ET cycles and 13 frozen ET cycles were subsequently completed.A total of 24 babies were born in 18 families,and the live birth rate per oocyte retrieval was 47.4 % （18/38）.No stillbirth or congenital malformation was observed in the translocation group.There was no significant difference in the pregnancy rate,miscarriage rate,implantation rate and live birth rate between the translocation group and control group.No significant difference was found between reciprocal translocation and Robertsonian translocation carriers.Conclusions：The clinical outcomes of IVF/ICSI of couples with paternal balanced translocation are comparable to those of chromosomally normal couples.Conventional IVF/ICSI is a feasible alternative for those patients.
