中西医结合治疗重症胰腺炎的临床研究

摘　　要：目的探讨中西医结合治疗重症胰腺炎的有效性。方法将75例急性重症胰腺炎病例分为单纯西医治疗组35例(对照组)和西医联合中医治疗组(治疗组)40例,比较两组患者症状消失、血尿淀粉酶、白细胞计数恢复正常所需时间,住院天数等。结果治疗组40例,1例死于多脏器衰竭,3例病情加重急诊行胰腺周围引流,胆结石胆囊切除术。对照组35例,3例死于多脏器衰竭,治疗组患者腹痛症状消失快,血尿淀粉酶及白细胞计数恢复正常需要时间短,住院时间短。结论在西药治疗基础上,结合中医药的通里攻下、疏肝解郁、清热解毒、逐瘀散结等个体化辨证施治,可有效抑制胰腺分泌,减轻胰腺的自身溶解和炎症反应,加速胰腺功能恢复,改善SAP的症状、体征,缩短病程,降低并发症及死亡率,提高治愈率。Objective To investigate the effectiveness of combination therapy of severe pancreatitis. Methods A total of 75 cases of severe acute pancreatitis patients were divided into western medicine group of 35 patients （control group） and western medicine combined therapy group （treatment group） 40 cases, two groups of patients symptoms, urine amylase, the time required to restore normal white blood cell count, days of hospitalization were compared. Results Of 40 patients in treatment group, 1 patient died of multiple organ failure, three cases of exacerbations emergency line drainage around the pancreas, gallstones cholecystectomy. Of 35 patients control group, 3 patients died of multiple organ failure, symptoms disappear quickly, hematuria, amylase and leukocyte counts returned to normal requires a short time, shorter hospital stay in treatment group. Conclusion On the basis of western medicine, combined with traditional Chinese medicine in the capture, liver qi stagnation, detoxification, and other individualized stasis stasis syndrome differentiation, which can effectively inhibit the secretion of the pancreas, reducing its own dissolution and pancreas inflammation accelerate the recovery of pancreatic function, improve SAP＇s symptoms, signs, shorten the course, reduce complications and mortality, improve the cure rate.
