摘　　要：目的评价Gelatamp胶质银明胶海绵预防后牙重度牙周炎患者拔牙后出现并发症的临床效果。方法以450例重度牙周炎患者共486颗需拔除的后牙为研究对象,根据患牙拔除后在牙槽窝内使用不同的材料,随机分成3组,试验组在牙槽窝内放置Gelatamp胶质银明胶海绵(A组),材料对照组则在牙槽窝内放置普通可吸收性明胶海绵(B组)以及空白对照组在牙槽窝内不放置任何材料(C组)。观察拔牙后0.5h、2d和7d时3组拔牙后牙槽窝的出血及感染的发生情况,采用SPSS17.0软件卡方检验来计算各组并发症的发生率。结果 3组拔牙后出血发生率分别为A组2.44%、B组5.56%、C组9.38%,感染发生率分别为A组1.22%、B组4.32%、C组7.50%,试验组拔牙后牙槽窝内出血及感染的发生率明显低于材料对照组及空白对照组,3组间的差异有显著性意义(P<0.05)。结论Gelatamp胶质银明胶海绵对后牙重度牙周炎患者拔牙后出现并发症的预防疗效显著,可在临床推广使用。Objective To study the clinical effects of Gelatamp colloidal silver gelatin sponge on preventing the complication of posterior teeth extraction with severe periodontitis. Methods 486 posterior teeth from 450 patients with severe periodentitis were divided into 3 groups randomly based on different materials in teeth extraction sockets： Gelatamp colloidal silver gelatin sponge was implanted into the teeth extraction sockets in the experimental group （Group A） . Gelatin sponge was implanted into sockets in the material control group（Group B） and nothing was implanted into the teeth extraction sockets in the blank control group （Group C）. The complication of teeth extraction was observed on 0.5 h, 2 d and 7 d after extraction. The incidence rate of complication was calculated by chi-square test using SPSS17.0 software. Results The incidence rate of bleeding in the three groups was 2.44% （Group A） , 5.56% （Group B） , 9.38% （Group C） respectively. And the incidence rate of infection in the three groups was 1.22% （Group A） , 4.32% rates of bleeding and infection complications in the （Group B） , 7.50% （Group C） respectively.
