固定-活动联合修复应用于咬合重建伴牙列缺损的临床观察

摘　　要：目的研究固定-活动联合修复在咬合重建伴牙列缺损治疗中的临床疗效。方法采用附着体义齿或套筒冠义齿为16例牙列缺损并需要咬合重建的患者进行修复,经过6个月至5年的临床观察,从患者主观感受、口腔临床检查、X线片检查3方面评价治疗效果。结果 16例患者主观感受稳固舒适,发音正常,对美观满意。临床检查16例患者颌位关系稳定,咬合良好;15例患者牙周情况稳定,基牙无松动,牙龈无红肿;1例出现基牙及阳性部件下牙龈红肿;15例患者基牙牙体情况良好,无叩痛,无折断;1例套筒冠义齿出现下切牙基牙折断。已完成8例X线片检查,8例颞颌关节无异常;8例基牙无根尖周病变;7例基牙无明显牙槽骨吸收,1例出现近缺隙侧基牙远中牙周膜增宽。结论固定-活动联合修复是咬合重建伴牙列缺损修复的一种有效方法。Objective The purpose of the study is to investigate the clinical effects of fixed-removable prostheses,which were applied to occlusal reconstruction and restoration of missing teeth. Methods Attachment-retained dentures and telescopic prostheses were applied to sixteen cases which need occlusal reconstruction and restoration of missing teeth. After six months to five years clinical observation,the therapeutic effects were evaluated basing on subjective feeling,oral examination and X-ray examination. Results Sixteen patients feeled good about the dentures. They were satisfied with the outlook and the pronunciation. According to oral examination,sixteen patients all had stable jaw relations.There were healthy periodontic situation of fifteen patients,there was no loose and swelling mucosa of the abutments.There were swollen gums under patrix and abutment of one patient. Only one patient had broken the down incisor abutment of the telescopic prostheses and there were excellent abutment conditions of the other fifteen patients. eight patients had checked by X-ray examination,they all had no abnormality of temporo-mandibular joints. There was no apical periodontitis of eight patients. There was no alvelor absorbsion of seven patients and there was widening of periodontal membrane of one patient. Conclusion Fixed-removable prostheses are effective to reconstruct occlusal relations and to restore missing teeth.
