摘 要:目的初步探讨声速匹配技术在乳腺良恶性病灶鉴别诊断中的临床价值。方法 73例患者共76个病灶(良性58个,恶性18个)行声速匹配值测定,计算声速,并与病理诊断结果对照,采用受试者工作特征(ROC)曲线确定良恶性病灶声速的临界值。结果良恶性病灶声速比较差异有统计学意义(P﹤0.05),ROC曲线确定诊断良恶性病灶的声速临界值为1578.33 m/s,ROC曲线下面积0.866,约登指数0.82,声速鉴别良恶性病灶的敏感性为85.7%,特异性为97.0%,准确性为94.7%。结论声速匹配技术可为乳腺病灶的临床诊断提供客观定量依据,声速临界值为超声鉴别诊断乳腺病灶良恶性提供了一项新指标。Objective To investigate the clinical value of sound speed correction technique in differential diagnosis of benign and malignant breast masses.Methods Seventy-three patients with 76 masses(58 benign, 18 malignant) were included in the study.The zone speed index was measured when the masses was found by the ultrasoud, and the sound velocity(SV)was calculated.Compared with the pathologic diagnosis,the threshold of SV was determined by the receiver operating characteristic (ROC) curve.Results There was significant difference of SV between benign and malignant masses (P 〈 0.05).The area of ROC curve was 0.866,Youden index was 0.82,sensitivity was 85.7% ,specificity was 97.0% ,and accuracy was 94.7%.Conclusion The sound speed correction technique could be used to provide objective quantitative proof for breast masses diagnosis, the threshold of SV could open a new diagnostic window in differential diagnosis of breast masses.
