北京地区宫颈病变妇女37种HPV亚型流行特征

摘　　要：目的探讨北京地区不同程度宫颈病变妇女中,37种HPV亚型感染流行情况。方法研究包括1 082例接受阴道镜检查及组织活检的宫颈病变患者和165名常规视野下宫颈正常的健康对照者,所有研究对象检测37种HPV基因型（PCR＋膜杂交法）,患者分为炎症和组织学异常（CIN 1、CIN 2/3和癌）组。结果宫颈炎妇女37种HPV亚型、高危型HPV的感染率分别为30.58%和19.22%,均显著低于组织学异常妇女（72.51%,χ2=6.763,P=0.009;66.79%,χ2=251.15,P=0.000）。HPV16、52、58、33是组织学异常妇女感染率最高的亚型。宫颈病变患者HR-HPV感染率在30岁-34岁和55岁-79岁年龄段表现出高峰。结论与其他人群相比,北京地区妇女感染HPV基因型不同,应对HR-HPV感染的高峰年龄人群进行密切观察。Objective To investigate the prevalence of 37 kinds of human papillomavirus （HPV） infection in women with cervical lesions in the Beijing region. Methods Specimens were collected from 1082 patients with abnormal cytology or cervical diseases and 165 normal controls without obvious cervical disease to detect the 37 kinds of HPV genotypes using a chip hybridization assay. All patients were also involved for colposcopic examination and biopsy. According to the results of histopathological diagnosis, patients were divided into normal histology group（cervicitis） and abnormal histology group（ CIN 1, CIN 2/3, cervical cancer）. Results The infection rates of 37 HPV subtypes were 30.58% and 72.51% in cervicitis group and abnormal histology group, respectively （X2 =6. 763, P =0. 009）, while the high - risk HPV infection rates were 19.22% and 66.79%, respectively （X2 = 251.15, P = 0.000）. HPV - 16, 52, 58, 33 were the most prevalent types in patients with abnormal cytology. There were two peaks of HR - HPV infection in cervical lesion patients, 30 - 34 years old and 55 - 79 years old. Conclusion Compared to prevalence rates of HPV infection in other populations, the prevalence of HPV genotypes varied a lot in women in Beijing region. It was suggested to give close observation on population at HR - HPV infection peak ages.
