腹主动脉瘤合并髂内动脉瘤的介入治疗  

Outcomes of Abdominal Aortic Aneurysm Accompanied with Internal Iliac Artery Aneurysm After Interventional Therapy

摘　　要：目的:探讨腹主动脉瘤合并髂内动脉瘤的介入治疗及其临床疗效。方法:对2011年10月-2013年3月应用弹簧圈栓塞髂内动脉后,行腔内隔绝术治疗腹主动脉瘤合并髂内动脉瘤的患者进行回顾性分析。其中男9例,女1例。结果:10例患者手术均获得成功,术中未发生瘤体破裂,无患者死亡,术后1周行CT增强复查,均无支架移位,无内漏,血流恢复正常。1例患者出现臀肌间歇性跛行,7例出现栓塞侧臀部胀痛以及会阴区域疼痛,给与镇痛治疗以及局部热敷治疗后1周,症状基本缓解或消失,无性功能障碍。结论:用弹簧圈栓塞髂内动脉瘤后行腔内隔绝术治疗腹主动脉瘤合并髂内动脉瘤是安全而有效的。Objective. To investigate the treatment effect of interventional therapy in abdominal aortic aneurysm accompanied with iliac aneurysms. Methods. From October 2011 to March 2013, ten patients with abdominal aortic aneurysm accompanied with iliac aneurysms, including 9 males and 1 female were retrospectively analysed. They were received coil embolization of internal iliac artery and then received endovascular graft exclusion. Results. All operations were successfully performed. No aneurysm rupture or death occurred during operation. One week later, all patients underwent CT enhancement scan, and no stent migration or endoleak was found. Only one case showed gluteal claudication. Three cases complained hip and perineal pain in embolization side, which were relieved after analgesic therapy and local hot compress therapy. Conclusion. Coil embolization and endovascular graft exclusion is a safe and effective treatment for abdominal aortic aneurysm accompanied with iliac aneurysms.
