超声内镜对胃微小间质瘤的诊疗价值

摘　　要：目的探讨超声内镜(EUS)对胃微小间质瘤(MGST)的诊疗价值。方法回顾性分析MGST30例患者的临床表现、常规内镜、超声内镜图像特征。将EUS的诊断结果与内镜治疗的病理组织学结果进行比较。结果胃微小间质瘤均为单发病灶,表面均无溃疡形成。好发于胃底(21/30,70%),其次位于胃体8例(8/30,26.7%)。EUS图像特征均为起源于固有肌层的均质低回声,向腔内生长。28例边界与浆膜层分界清晰(93.3%),2例边界与浆膜层关系密切(6.7%)。平均直径(7.0±1.6)mm。所有患者均行内镜下切除瘤体,其中28例行内镜下黏膜剥离术(ESD),2例行内镜下全层切除术(EFR)。结论超声内镜检查是判断胃微小间质瘤范围及指导治疗的重要手段。Objective To evaluate endoscopic uhrasonography （EUS）as used in the diagnosis and treatment of minute gastric stromal tumor. Method We reviewed and analyzed the dates of 30 patients who pathologically confirmed minute gastric stromal tumor. Result Minute gastric stromal tumors were single, no ulcer on the surface. They were frequently found in the fundus（21/30,70% ）, then in the body（8/30,26.7% ）. In all of the cases（30/30,100% ） ,the lesions originated from the muscularis propria with hypo-echo. Most cases had a clear outline （28/30,93.3 % ）with the serosa. The average diameter was（7.0 ±1.6）mm. All of the lesions were removed by endoscopy, in which 28 cases by EMR, 2 cases by EFR. Conclusion EUS was a useful tool for the diagnosis and treatment of minute gastric stromal tumor.
