摘 要:目的观察Smad4和转化生长因子(TGF)-β1在膀胱癌组织内的表达情况,分析Smad4和TGF-β1的表达与膀胱癌进展及预后之间的关系。方法选取临床资料完整的膀胱癌病例48例,其中,淋巴结转移组20例,无淋巴结转移组28例。免疫组化法检测膀胱癌组织内Smad4和TGF-β1的表达。取膀胱癌术后组织16例,Western blot法检测Smad4和TGF-β1在膀胱癌组织内的表达情况。结果 Smad4表达于肿瘤细胞浆和胞核,在有淋巴结转移膀胱癌组织内的表达量明显低于其在无淋巴结转移组的表达量,Smad4表达与膀胱癌临床分期、分级、淋巴结转移以及膀胱癌复发密切相关(<0.05)。TGF-β1表达于肿瘤细胞的胞浆内,在有淋巴结转移膀胱癌组织中的表达量高于其在无淋巴结转移膀胱癌组织内的表达量,TGF-β1的表达与膀胱癌临床分期、分级及淋巴结转移之间具有相关性(<0.05)。Smad4和TGF-β1在膀胱癌组织内的表达呈显著负相关(r=-0.324,=0.025)。Kaplan-Meier分析表明Smad4阳性患者的五年总体生存率高于Smad4表达阴性患者的生存率,而TGF-β1的表达与膀胱癌患者的预后无关。结论 Smad4和TGF-β1在膀胱癌组织的表达呈负相关,与膀胱癌临床分期、分级及淋巴结转移相关。Objective To investigate the effect of Smad4 and TGF-β1 on tumor progression and clinical prognosis, we observed the expression of Smad4 and TGF-β1 in bladder cancer. Methods Forty-eight paraffinembedded stored specimens from patients with transitional bladder cancer were included in this study. Among all of 48specimens, they were divided into lymph node metastatic group(n=20) and nonmetastatic group(n=28). The expressions of Smad4 and TGF-β1 were detected by immunohistochemical stain and Western blot. Results Smad4 protein product was observed in cytoplasm and nucleus of tumor cells, the expression level was lower in lymph node metastatic group than in nonmetastatic group, the expression of Smad4 was related to clinical stage, pathological grade, lymph node metastasis and recurrence in bladder cancer( 〈0.05). TGF-β1 protein product was observed predominantly in the cytoplasm of the tumor cells, the expression level was higher in lymph node metastasis than in nonmetastasis, the expression of TGF-β1 was closely correlated with clinical stage, pathological grade and lymph node metastasis( 〈0.05). Smad4 expression in bladder cancer showed negative correlation with the expression of TGF-β1(r=-0.324, =0.025). Kaplan-Meier model analysis indicated that 5-year overall survival rate was higher in Smad4 positive patients than in Smad4 negative patients. None correlation was found between TGF-β1 expression and prognosis. Conclusion The expression of Smad4 was closely related to TGF-β1 expression negatively, they were closely related to clinical stage, pathological grade and lymph node metastasis.
