rFVIIa成功控制合并肾衰呼吸衰竭的消化道大出血1例

摘　　要：目的探讨消化道大出血的治疗。方法重组人凝血因子VIIa(rFVIIa)是一种凝血因子制剂,国外文献报道可控制危及生命的出血。1例78岁的消化道大出血患者合并肾衰呼吸衰竭,经过常规药物治疗无效,出血持续,出现休克。应用rFVIIa 20μg/kg静脉注射治疗。结果出血迅速控制,生命体征逐渐稳定。结论对于常规方法无法控制的消化道大出血,rFVIIa可作为尝试治疗。Recombinant activated factor VII （rFVIIa） is a novel therapeutic agent for life-threatening massive gastrointestinal bleeding. We report a case of massive gastrointestinal bleeding in a 78-year-old female patient with respiratory and renal failure. After failure of management of the bleeding with routine pharmacotherap35 we gave the patient rFVIIa injection at the close of 20 μg/kg and the bleeding was rapidly controlled. Adverse side effects of the drug were not observed in this patient.
