摘 要:目的:筛查胃肠间质瘤(GIST)住院病人的营养风险发生情况。方法:对在我院胃肠外科住院的235例GIST病人进行营养风险筛查,按照肿瘤危险性将其分为极低危险性组、低危险性组、中危险性组和高危险性组;按肿瘤发生部位分胃、十二指肠、小肠、结肠、直肠、系膜和腹腔其他部位等调查,并比较各组营养风险发生率。结果:235例病人均完成NRS 2002筛查,营养不足和营养风险的发生率分别为55例(23.40%)和80例(34.04%),其中极低危险性组病人营养风险发生率为28.57%、低危险性组为22.41%、中危险性组为25%、高危险性组为50%。组间比较发现,高危险性组病人营养不良风险性明显高于其他组(P<0.05);按照肿瘤部位分析病人营养风险发生率,胃间质瘤为28.15%、十二指肠42.85%、小肠35.89%、结肠18.75%、直肠37.5%、系膜75%、腹腔其他部位71.42%。系膜和腹腔其他部位肿瘤病人营养不良风险性明显高于其他组(P<0.05)。结论:GIST住院病人存在一定的营养风险,高危险性和系膜、腹腔其他部位肿瘤病人较其他GIST病人营养风险发生率更高。Objective:Assessment of nutritional risk status was made among hospitalized patients with gastrointestinal stromal tumor(GIST).Methods:The nutritional risk features of 235 GIST cases were collected.All GIST cases were classified into very low,low,intermediate and high-risk groups respectively according to the histological risk classification.Nuritional risk was also evaluated and compared according to tumor location.Results:All patient's assessments of nutritional risk screening were completed.The incidences of undernutrition and nutritional risk were 23.40% (55 cases) and 34.04% (80 cases).The nutritional risk incidence in very low,low,intermediate and high-risk GIST groups were 28.57%,22.41%,25% and 50%,respectively.The high-risk group had significantly higher proportion of nutritional risk than the other three groups (P < 0.05).According to the tumor location,the incidences of nutritional risk for stomach,duodenum,small intestine,colon,rectum,mesenteric and other parts of abdominal cavity were 28.15%,42.85%,35.89%,18.75%,37.5%,75% and 71.42%,respectively.The incidence of nutritional risk amongst patients in the last two groups was significantly lower than that in the other groups (P < 0.05).Conclusion:There is convincing evidence that nutritional risk is prevalent among GIST patients.GIST patients with high-risk and in mesenteric and other parts of abdominal cavity have the highest incidences of nutritional risk.
