摘 要:目的通过测量Ⅱ类骨面型前牙深覆盖患者矫治前后颌面部特征指标,比较其矫治前后颌骨,牙和牙槽特征指标的变化情况。方法对53例Ⅱ类骨面型前牙深覆盖患者按其矫治设计不同分为非拔牙矫治组(19例)和拔除4个第一前磨牙矫治组(34例),共选择14项测量指标,分别对其矫治前后头颅定位侧位片进行比较(t检验),分析其矫治效果。结果 119例Ⅱ类骨面型前牙深覆盖非拔牙患者矫治前后比较显示U1-SN、L1-SN、L1-MP、Co-Po、ANS-Me、OJ、SNB、ANB有统计学意义(P<0.05)。234例Ⅱ类骨面型前牙深覆盖拔牙患者矫治前后比较显示U1-SN、L1-SN、L1-MP、U1-NA、L1-NB、ANS-Ptm、Ptm-6、Co-Po、N-Me、ANS-Me、N-ANS/N-Me、ANS-Me/N-Me、OJ、SNB、SNA、ANB有统计学意义(P<0.05)。结论Ⅱ类骨面型前牙深覆盖患者矫治前后比较显示在拔牙与非拔牙病例中上下前牙位置、下颌长度、下面高、覆盖及下颌骨位置都得到明显改善,但在拔牙病例中上颌骨位置、全面高改善显著。因此深覆盖患者伴有上颌前突或面高发育不足者应考虑拔牙矫治。Objective The aim of this study was the measurement of indexes of maxillofacial features of class II skeletal facial type front teeth deep overjet patients before and after treatment, and to compare the changes of jaw, teeth and alveloar character indexes before and after treatment. Methods According to different designs, 53 cases of class Ⅱskeletal facial type front teeth deep overjet patients were divided into non-extraction treatment group(19 cases) and unplug four first premolars treatment group(34 cases), 14 measurement indexes were selected, to compare cephalometric radiographs(t test)before and after treatment respectively, and to analyze the treatment effects. Results 1Comparisons of 19 cases of class Ⅱskeletal facial type front teeth deep overjet tooth non-extraction patients before and after treatment showed that U1-SN, L1-SN, L1-MP, Co-Po, ANS-Me, OJ, SNB, ANB were statistically significant(P 0.05). 2Comparisons of 34 cases of type Ⅱskeletal facial type front teeth deep overjet tooth extraction patients before and after treatment showed that U1-SN, L1-SN, L1-MP, U1-NA, L1-NB, ANS-Ptm, Ptm-6, Co- Po, N-Me, ANS-Me, N-ANS/NMe%, ANS-Me/N-Me%, OJ, SNB, SNA, ANB were statistically significant(P〈0.05). Conclusion Comparisons of ClassⅡ skeletal facial type front teeth deep overjet patients before and after treatment showed that tooth extraction and nonextraction cases, the upper and lower front teeth position, mandibular length, lower facial height, coverage and mandibular position have been significantly improved, but in cases of tooth extraction, maxillary bone position, the whole facial height have been significantly improved. Therefore, patients with deep coverage maxillary protrusion or facial height hypoplasia should consider tooth extraction orthodontics treatment.
