胸锁乳突肌瓣联合异种脱细胞真皮基质预防味觉出汗综合征

摘　　要：目的观察在腮腺部分切除术中联合应用胸锁乳突肌瓣填塞和脱细胞真皮基质覆盖重建腮腺咬肌筋膜对味觉出汗综合征及面部凹陷畸形的预防效果。方法 15例腮腺良性肿瘤患者行瘤体及瘤周腮腺区域性切除术,同期行胸锁乳突肌瓣转移修复及异种脱细胞真皮基质口腔修复膜腮腺咬肌筋膜重建术。分别在术后6个月、12个月随访,采用咀嚼酸性食物或维生素C片诱导碘-淀粉试验,观察术后味觉出汗综合征和面部凹陷畸形的发生情况。结果全部患者无1例发生味觉出汗综合征和面部凹陷畸形。结论联合应用胸锁乳突肌瓣填塞和脱细胞真皮基质重建腮腺咬肌筋膜对预防味觉出汗综合征及面部凹陷畸形效果肯定。Objective To prevent Frey＇ s syndrome in parutidectomy, stenrocleidomastoid /lap filling and acelhdar dermal natrix covering were used to reconstruct the fascia parotideomassetaefica. Methods Fifteen cases of benign pa- rotid tumor patients were treated for extended resection and partial resection of the parotid gland, and parotideomassetaerica facial flap with heterogeneous acellular dermalmatrix were used for reconstruction. After 6 months, 12 months followup, acidic food or vitamin C tablets and iodine-starch test were used to observe the occurrence of postoperative Frey＇ s syndrome and facial deformity. Results No skin flushing, sweating or facial deformity was observed in these patients. Conclusion Reconstruction of fascia parotideomasseterica by Stenroclei-domastoid flap filling and acellular dermal matrix covering has a remarkable effect on preventing the occurrence of Frey＇ s syndrome and facial deformity.
