摘 要:目的:分析基质金属蛋白酶2(MMP-2)和MMP-9及其抑制因子1(TIMP-1)、TIMP-2与宫颈癌浸润转移的关系。方法:选取经病理证实的宫颈浸润癌患者108例(ICC组)、宫颈上皮内瘤样病变患者65例(CIN组);同时,选取正常宫颈组织标本40例为对照组。应用逆转录-聚合酶链反应(RT-PCR)检测各组组织中MMP-2、MMP-9、TIMP-1和TIMP-2表达水平。比较各组相关因子表达水平差异,并分析ICC组各因子表达水平与临床病理参数的关系。结果:ICC组MMP-2和MMP-9的表达水平显著高于CIN组和对照组,TIMP-2和TIMP-1的表达水平显著低于CIN组和对照组(P<0.05)。ICC组的MMP-2、MMP-9、TIMP-1和TIMP-2的表达水平与FIGO分期、组织学分级、分化程度、脉管浸润、淋巴结转移和浸润深度有关(P<0.05),与大体类型和组织类型无关(P>0.05)。结论:MMP-2、MMP-9及其抑制因子TIMP-2、TIMP-1可能在宫颈癌的侵袭与转移中发挥重要作用,可作为评估宫颈癌侵袭转移潜能的重要指标。Objective: To investigate the correlation of matrix metalloproteinase (MMP-2, MMP-9) and its inhibitors (TIMP-2, TIMP-1) with the invasion and metastasis of cervical cancer. Methods: 108 patients with early invasive cervical cancer(ICC) and 65 patients with cervical intraepithelial neoplasm(CIN) confirmed by pathology were selected, and 40 cases of normal cervical epithelium(NCE) were selected as control. The expression of MMP-2, MMP-9, TIMP-2 and TIMP-1 in all samples were examined by reverse transcription PCR. Expression levels of each factor in different groups were compared. The correlations between expression levels of each factor and clinical pathological parameters in ICC group were analyzed. Results: Compared with control group and CIN group, expression levels of MMP-2, MMP-9 in ICC group were significantly higher, TIMP-2 and TIMP-1 were significantly lower (P 〈 0.05 ). In ICC group, expression levels of MMP-2, MMP-9, TIMP-2, TIMP-1 were correlated with FIGO clinical stage, histological grading, differentiated degree, intravascular and stromal infiltration and lymph node metastasis (P 〈 0.05) , but not correlated with general types and histological types(P 〉 0.05). Conclusion: MMP-2, MMP-9 and TIMP-2, TIMP-1 may play an important role in invasion and metastasis of cervical cancer, and could be used to estimate the potential metastasis.
