P16与Ki-67在恶性黑色素瘤中的表达及临床病理意义

摘　　要：目的探讨P16与Ki-67在恶性黑色素瘤和皮内痣中的表达及鉴别诊断意义。方法选取该院2005年5月至2012年8月病理诊断为恶性黑色素瘤的患者31例,所有患者由2位主任医师再次复查确诊。同时选取30例皮内痣患者作为对照。应用免疫组织化学法检测恶性黑色素瘤及皮内痣组织中P16与Ki-67蛋白表达情况,并分析其与临床病理参数的关系。结果在恶性黑色素瘤组织中P16与Ki-67蛋白阳性表达率与皮内痣组织比较,差异有统计学意义(P<0.05);P16蛋白的阳性表达与浸润深度及淋巴结转移有关(P<0.05),与其他病理参数无关(P>0.05);Ki-67蛋白的阳性表达与临床病理参数均无关(P>0.05)。P16的表达与Ki-67的表达呈负相关(r=-0.453,P<0.05)。结论联合检测P16与Ki-67对恶性黑色素瘤和皮内痣的鉴别诊断具有重要的意义。Objective To explore the expression and diagnostic value of P16 and Ki-67 in malignant melanoma and intradermal nevus. Methods Thirty-one patients,who were diagnosed with malignant melanoma in the hospital from May 2005 to August 2012, were selected and reexamined by two chief physicians. Meanwhile other 30 patients with intradermal nevus were selected as control group. Immunohistochemical method was used to detect the protein expression of P16 and Ki-67 in malignant melanoma and intradermal nerus,and analyze the relationship with clnicopathological parameters. Results There were statistically significant differences in positive rate of P16 protein and Ki-67 protein between malignant melanoma tissue and intradermal nerus tissue（P〈0.05）. The expression of P16 protein was correlated with depth of invasion and lymphatic metastasis（P〈0.05）,but there was no correlation between expression of P16 protein and other pathological parameters（P〉0.05）. There was no correlation between expression of Ki-67 protein and other clinicopathological parameters（P〉0.05）. There was a negative correlation between P16 expression and Ki-67 expression（r=-0.453,P〈0.05）. Conclusion Combined detection of P16 and Ki-67 has vital significance for the differential diagnosis of malignant melanoma and benign nevus melanin.
