经尿道电切术治疗成人输尿管囊肿的有效性和安全性

摘　　要：目的评价经尿道电切术治疗输尿管囊肿的有效性和安全性。方法回顾性分析我院2001年1月至2012年12月共52例经尿道电切术治疗的输尿管囊肿患者的临床资料。囊肿位于左侧28例,右侧14例,双侧10例,9例伴囊肿内结石,2例伴同侧肾输尿管重复畸形。采用经尿道电切术行输尿管囊肿切除术,术时保留上方囊壁作为抗返流的活瓣;合并结石患者采用大力钳或钬激光碎石。结果 52例患者手术均一次成功。手术平均时间30 min,术后平均住院3天。术后随访6个月至10年,定期行B超和静脉尿路造影(intravenous urography,IUV)。患者未见肾积水和输尿管扩张,临床症状消失,无膀胱-输尿管返流发生,无输尿管囊肿复发。结论经尿道电切术是治疗输尿管囊肿的一种简便、有效、安全的微创治疗方法。Objective To evaluate the efficacy and safety of endoscopic incision of orthotopic ureterocele in adults.Methods From Jan.2001 to Dec.2012,52 patients underwent transurethral endoscopic incision of ureterocele in Huashan Hospital.Unilateral ureterocele was presented in 42 patients （left 28 and right 14）,and 10 patients had bilateral ureterocele.Nine patients had calculus in ureterocele,and 2 patients had deformity of ureter in unilateral kidney.Transurethral endoscopic incision was used to excise ureterocele in all patients,and the upper cyst wall was maintained during the surgery as the valve against ureteral reflux.The stones were removed using locking pliers and holmium laser.Results All surgeries were successful at the first time.The mean operative time and hospital stay were 30 minutes and 3 days,respectively.Follow-up ranged from 6 months to 10 years.Postoperative regularly ultrasound and intravenous urography （IUV） revealed no uronephrosis and ureterectasis in all patients,free of symptom,ureteral reflux and dinical recrudescence.Conclusions Management with endoscopic incision of orthotopic ureterocele is effective and safe.
