临时冠对上颌单颗前牙即刻种植义齿周围软组织成形的影响

摘　　要：目的研究种植体支持的临时冠对于上颌单颗前牙种植体周围软组织成形的美学效果的影响。方法采取病例对照研究，选取36例上颌单个前牙即刻种植的患者，根据临时修复的方式随机分为2组，实验组在种植体植入后采用种植体支持的临时冠修复，对照组在种植体植入后采用局部可摘义齿进行临时修复。分别在种植义齿最终修复完成时，以及种植义齿行使功能一年后，采用粉红美学指数对种植义齿周围的软组织变化进行美学评估。结果36例种植体全部成功，在种植义齿最终修复完成时，两组的软组织美学得分有显著性差异，而在种植义齿行使功能一年后，两组的软组织美学得分没有显著性差异。结论种植义齿支持的临时冠修复对于早期完成种植体周围的软组织美学成形有很好的作用，但是种植义齿周围软组织美学效果最终还是取决于种植体植入的深度、种植体周围软硬组织的生物学特性。因此，在选择种植治疗时，一定要严格把握适应症，同时采用多种方法改变种植区的软硬组织特性，以达到更好的美学效果。Objective To explore the effect of soft tissue aesthetic outcomes following the implant supported temporary crown in maxillary single anterior implant denture. Methods 36 patients who would undergo implant placement in the anterior maxilla were randomly divided into two groups, experimental group in which implant supporting temporary crown was used as the temporary repair, and the control group in which partial removable denture was used as the temporary repair. The aesthetic outcome of the soft tissue was assessed immediately at completion of the permanent crown procedure and 1 year after. Results Although the 36 implants all succeeded, there was a significant difference in aesthetic evaluation between the two groups after the permanent crown was finished. However, the difference disappeared after one year of the procedure. Conclusion Implant supporting temporary crown is effective in soft tissue forming at early stage. However, the aesthetic outcome of the implant denture ultimately depends on the depth of the implant, and the biological characteristics of the hard and soft tissues. If implant treatment, in chosen it is important to give primary attention to all the indications, simultaneously, adopting multiple methods to change the area of soft tissue features in order to achieve better aesthetic effect.
