摘 要:目的探讨肿瘤标志物血清癌胚抗原(CEA)、糖类抗原125(CA125)和端粒酶活性联合检测在卵巢癌早期诊断中的临床意义。方法采用改进的银染-TRAP(端粒重复序列扩增)法检测43例卵巢癌患者和45例正常对照者外周血端粒酶活性,同时用电化学发光免疫法对其血清CEA、CA125进行了测定。结果卵巢癌组与对照组比较,卵巢癌组血清CEA、CA125的含量均明显高于对照组,差异有显著性(P<0.01);卵巢癌组端粒酶活性阳性率为62.8%,对照组为4.4%,差异有显著性(P<0.01);卵巢癌单项检测阳性率:CEA为32.6%,CA125为76.7%,端粒酶活性为62.8%,三项联合检测可提高卵巢癌的灵敏度至90.7%,明显高于单项检测的灵敏度。结论肿瘤标志物CEA、CA125、端粒酶活性联合检测可以显著提高卵巢癌诊断的灵敏度,为处于亚临床期的卵巢癌患者及正常体检人群中卵巢肿瘤的早期检出提供了可靠的检测方法。[ Objective ] To explore the diagnostic value of combined detection of blood serum carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), and telomerase activity in ovarian carcinoma. [Methods] Using the improved-TRAP (silver staining telomeric repeat amplification protocol) assay in 43 cases of patients with ovari- an cancer, 45 cases of normal peripheral blood telomerase activity, at the same time with electrochemiluminescence immuuoassay for serum CEA, CA125 were determined. [Results] Ovarian cancer group and control group, the content of serum CEA, CA125 ovarian cancer group were significantly higher than the control group, the difference was statistically significant (P 〈0.01); the positive rate of telomerase activity of ovarian cancer group was 62.8%, 4.4% in the control group, the difference was statistically significant (P 〈0.01), the positive rate of single detection of ovarian cancer: CEA 32.6%, CA125 is 76.7%, the telomerase activity was 62.8%, the three combined detection can improve the sensitivity of ovarian cancer to 90.7%, significantly higher than the single detection sensitivity. [ Conclusions ] Tumor markers CEA, CA125, telomerase activity detection can significantly improve the sensitivity for the diagnosis of ovarian cancer, in sub clinical stage of ovarian cancer patients and normal physical examination crowd ovarian cancer early detection provides a reliable detection method.
