摘 要:目的分析肝炎肝硬化患者的FibroScan检测值(FS值)与面部色泽变化的相关性,为中医望色判断病情严重程度和病机分析提供客观依据。方法将108例肝炎肝硬化患者划分为代偿期69例,失代偿期39例。运用《肝炎肝硬化临床信息采集表》分别收集两期患者的面色、FS值等,运用SPSS 13.0软件进行描述性统计分析、t检验、单因素方差分析等。结果 108例肝炎肝硬化患者,FS值最小值4.1KPa,最大值75KPa,平均(24.07±18.43)KPa,其中代偿期(16.18±10.06)KPa,失代偿期(38.03±21.50)KPa,两期比较有统计学差异(P<0.01);108例肝炎肝硬化患者正常面色、面红、淡黑、淡黄、淡白、淡青色在代偿期出现比例较高,黝黑、黄黑、青黑色在失代偿期出现比例较高;不同面色的FS值比较差异有统计学意义(P<0.05);面色晦暗程度与FS值变化进行分析,发现面色晦暗程度越重,FS值越来越高,组间比较差异有统计学意义(P<0.01)。结论作为判断病情严重程度的FS值与面部色泽变化有一定的相关性,面色越黑或色泽越暗,FS值越高,而且面色黑在肝硬化多提示血瘀,说明肝硬化病情越重,血瘀越重。Objective To analyze the correlation of FibroScan (FS) value and complexion changes in posthepatitic cirrhosis patients and provide objective basis for judging the condition and analyzing pathogenesis according to inspection of the complexion. Methods Totally 108 cases of posthepatitic cirrhosis were divided into 69 cases of compensation and 39 cases of decompensation. The FS values and complexion changes were collected with Clinical Information Collection Form of Posthepatitic Cirrhosis. The descriptive statistical analysis, t test and one-way ANOVA were conducted with SPSS 13.0 software. Results The minimum, maximum and average FS values were 4. 1 KPa, 75 KPa and (24. 07± 18.43 ) KPa respectively. The average FS values of compensation and deeompensation were (16. 18 ± 10. 06) KPa and (38.03 ±21.50) KPa respectively, with a significant difference between the compensa- tion and decompensation (P 〈 0.01 ). The normal complexion, red complexion, light black complexion, light yellow complexion, pale complexion and pale blue complexion were commonly seen in the compensation. The dark complexion, yellow black complexion and blue black complexion were commonly seen in the decompensation. There was a significant difference between different complexion in FS value (P 〈 0. 05 ). The darker complexion showed, the higher FS value was increased, with a significant difference between groups (P 〈 0. 01 ). Conclusion FS value has a certain correlation with complexion changes. The darker complexion shows, the higher FS value is increased. Dark complexion in cirrhosis indicates blood stasis.
