摘　　要：目的探讨浸润性导管癌（IDC）超声造影血流动力学特征与Ki-67表达之间的相关性。方法回顾性分析经手术切除后病理确诊的85例IDC患者的109个肿块。根据Ki-67免疫组化结果分为阴性组（-）、弱阳性组（＋）、阳性组（＋＋），强阳性组（＋＋＋）。通过超声造影比较每两组间的血流动力学参数。结果“＋＋＋”组分别与“-”组、“＋”组及“＋＋”组相比，达峰时间、始增强度、峰值强度均显著升高，差异具有统计学意义；“-”组分别与“＋＋”组、“＋”组相比，始增强度、峰值强度均显著升高，差异具有统计学意义。Ki-67表达与始增强度、强度差值和上升支斜率呈正相关，与始增时间、达峰时间和渡越时间呈负相关。其余组问两两比较中，始增时间、始增强度的差异均有统计学意义。结论超声造影血流动力学参数定量分析与Ki-67有较好的相关性，可作为临床诊断IDC的有效补充。Objective To investigate the correlation between the hemodynamic characteristics of contrast enhanced ultrasound and the expression of antigen Ki-67 in infiltrative ductal carcinoma （IDC）. Methods 109 masses founded in 85 cases were examined by contrast enhanced ultrasound before resection and diagnosed of infiltrative ductal carcinoma through pathological test after resection were included. According to the results of immunohistochemical examination aiming at the expression of antigen Ki-67,all masses were divided into negative group, weakly positive group, positive group and strong positive group. Compared to the hemodynamic parameters of every two groups during the contrast enhanced ultrasonography, the results were analyzed statistically. Results Compared to negative group, weakly positive group and positive group, strong positive group had significant statistically differences in time to peak,arrive intensity and peak intensity. Compared to other three groups, the negative group had the significant statistically differences in arrive intensity and peak intensity. The antigen Ki-67 had positive correlation with arrived intensity, intensity changes, rising curve＇ s slope, and negative correlation with arrived time, time to peak, continuous time. The differences of arrived time and arrived intensity were significant statistically in each two groups.Conclusions The hemodynamic parameters of contrast enhanced ultrasonography has a good correlation with the expression of antigen Ki-67,it is an effective supplement to diagnosis IDC.
