PKRBt术中黏膜下多点注射联合表柔比星灌注治疗表浅性膀胱癌的临床效果

摘　　要：目的：探讨PKRBt（经尿道等离子膀胱肿瘤切除术）术中黏膜下多点注射联合表柔比星灌注治疗表浅性膀胱癌的临床应用价值。方法2012年3月至2013年12月63例符合手术治疗指征的表浅性膀胱癌患者，手术均采用PKRBt术中黏膜下多点注射，术后即刻或者24h之内辅以表柔比星膀胱灌注。结果手术均顺利完成，手术操作时间30～55 min，术中并发闭孔神经反射4例，1例术后出现膀胱填塞，未出现膀胱穿孔等并发症。随访3个月～2年，其中8例复发，再次进行相同手术，未出现复发。结论 PKRBt术中黏膜下多点注射联合表柔比星灌注化疗治疗表浅膀胱肿瘤疗安全性高，复发率低，并发症少，具有很好的临床应用价值。Objective To investigate the clinical application of PKRBt intraoperative submucosal multipoint injection combined with epirubicin instillation in the treatment of superficial bladder cancer .Methods We retrospectively reviewed the clinical datas of 63 patients who hospitalized in our hospital during March 2012 to December 2013, they suffered from superficial bladder cancer and meet the indications of surgical treatment , they received the operation of PKRBt intraoperative submucosal multipoint injection combined with epirubicin instillation.Results All operations were successfully completed , the operation time was 30～55 min.Obturator nerve reflex occurred in four cases during the operation , one case of postoperative bladder filling .None bladder perforation appeared .The patients were followed up for 3 months to 2 years, eight cases of them got recurrence , and they received the same operation again without recurrence .Conclusions PKRBt intraoperative submucosal multipoint injection combined with epirubicin instillation for the treatment of superficial bladder cancer has the advantages including safe , low recurrence rate , fewer complications , worth of clinical practice .
