尼莫地平联合奥拉西坦用于高血压脑出血患者认知恢复临床观察

摘　　要：目的观察尼莫地平联合奥拉西坦治疗高血压脑出血患者认知功能恢复的临床疗效。方法将66例高血压脑出血患者随机分为试验组和对照组,对照组患者给予常规基础治疗,试验组患者在对照组基础上给予尼莫地平联合奥拉西坦治疗。治疗前、后进行CT扫描测定血肿和水肿体积,应用美国国立卫生研究院卒中量表(NIHSS)、简易精神状态表(MMES)、巴氏指数评分(BI)评价临床疗效。结果治疗后,两组患者血肿和水肿体积均减少,与治疗前比较差异有显著性(P<0.05),且试验组与对照组比较差异有显著性(P<0.05);治疗后,两组患者NIHSS评分减少,MMES和BI评分升高,与治疗前比较差异有显著性(P<0.05),且试验组与对照组比较差异有显著性(P<0.05)。结论尼莫地平联合奥拉西坦治疗高血压脑出血效果显著,能有效减轻脑血肿和水肿状况,改善患者的神经功能和认知功能。Objective To investigate the clinical effects of Nimodipine combined with Oxiracetam in patients with hypertensive cerebral hemorrhage. Method 66 cases of patients with hypertensive cerebral hemorrhage were randomly divided into treatment group and control group, the control group was given routine treatment and individual therapy, the treatment group was given Nimodipine combined with Oxiracetam treatment based on the control group. The volume of hematoma and perilesional edema of the two groups were determinated by using CT scan. The clinical effect was evaluated by the national institutes of health stroke scale（NIHSS） scores and the Barthel index（BI） scores and the mini mental state examination（MMES） scores before and after the treatment. Result After treatment, the volume of hematoma and perilesional edema of the two groups decreased significantly, compared with pre-treatment the difference was significant（P〈0.05）, and the treatment group compared with the control group the difference was significant（P〈0.05）. The two groups included decreased NIHSS and elevated MMES and BI after treatment, compared with pre-treatment the difference was significant（P〈0.05）, and the treatment group compared with the control group the difference was significant（P〈0.05）. Conclusion Nimodipine combined with Oxiracetam is an effective treatment for hypertensive cerebral hemorrhage, can effectively reduced the size of edema zone around the hemotoma and improve the neurological function and cognitive function.
