外周血单个核细胞Tim-3/Tim-1mRNA比值失衡对桥本甲状腺炎癌变的诊断意义

摘　　要：目的 分析外周血单个核细胞Tim-3/Tim-1 mRNA比值失衡对桥本甲状腺炎（HT）癌变的诊断意义.方法 92例桥本甲状腺炎,其中24例HT合并癌变,另取22例单纯性甲状腺癌患者外周血和38例正常人的外周血作为对照,采用RT-PCR法分别检测所有外周血单个核细胞中Tim-3/Tim-1 mRNA比值的情况.结果 HT组,HT癌变组,甲状腺癌组和正常对照组中外周血单个核细胞Tim-3/Tim-1 mRNA比值有显著性差异.HT组的Tim-3/Tim-1[（0.91±0.38） copies/μL]低于对照组（P＜0.05）,HT癌变组的Tim-3/Tim-1[（0.43±0.26） copies/μL]低于HT组和对照组（P＜0.05）,甲状腺癌组[（0.81 ±0.48） copies/μL]的Tim-3/Tim-1和对照组差别异无统计学意义（P＞0.05）.结论 本研究结果提示外周血Tim-3/Tim-1比值失衡在对诊断HT及其癌变有重要的临床意义,证实HT是一种癌前病变,Tim-3/Tim-1比值失衡可能是HT癌变的标志.Objective To study the unbalanced Tim-3/Tim-1 mRNA ratio in peripheral blood mononuclear cell and its diagnostic values in cancerization for hashimoto thyroiditis （HT）patients.Methods There were 92 cases of HT and 24 cases of them were combined with cancerization.Peripheral blood from 22 cases of thyroid cancer and 38 cases of normal people was obtained as controls.Tim-3/Tim-1 mRNA ratio in peripheral blood mononuclear cell was tested with RT-PCR in all blood samples.Results There were significant differences in Tim-3/Tim-1 mRNA ratio between the four groups.Tim-3/Tim-1 ratio in the simple HT group（0.91 ± 0.38 copies/μL） was lower than that of the normal control group（P ＜ 0.05）,and in the cancerization group,the ratio（0.43 ± 0.26copies/μL） was lower than that of the simple HT and normal control group （P ＜ 0.05）.There was no significance between the thyroid cancer group （0.81 ± 0.48copies/μL） and the normal control group（P＞0.05）.Conclusion Unbalanced Tim-3/Tim1 mRNA ratio in peripheral blood is valuable for the diagnosis of HT and its cancerization.HT is one of the precancerous lesions and unbalanced Tim-3/Tim-1 mRNA ratio in peripheral blood may be a marker for the cancerization.
