个案管理对慢性精神分裂症患者生活质量及社会功能的影响

摘　　要：目的探讨个案管理对慢性精神分裂症患者生活质量及社会功能的影响。方法对蓬江、江海两区150例慢性精神分裂症患者进行个案管理服务,并在开始个案管理服务前、个案管理服务24个月后分别用阳性与阴性症状量表(PANSS)、自知力与治疗态度问卷(ITAQ)、精神分裂症病人生活质量量表(SQLS)和个人与社会表现量表(PSP)进行评估。结果个案管理前后,PANSS阳性症状量表评分、阴性症状量表评分、一般精神病理症状量表评分、总评分和ITAQ评分[(15.42±6.72)vs.(12.13±4.27),(25.44±6.72)vs.(22.56±6.46),(38.93±8.22)vs.(33.58±7.84),(79.79±18.31)vs.(68.26±16.53),(10.08±4.57)vs.(11.7±5.25)]差异均有统计学意义(P均<0.05或0.01),SQLS心理社会、精力/动力、症状/副反应因子分、SQLS总评分及PSP评分[(18.13±7.27)vs.(13.06±6.07),(12.05±3.21)vs.(9.83±2.79),(7.68±2.95)vs.(29.13±9.45),(37.86±11.31)vs.(29.13±9.45),(46.82±12.87)vs.(52.26±12.76)]差异也均有统计学意义(P均<0.01)。结论对慢性精神分裂症患者实施个案管理服务,可有效控制患者病情,提高患者生活质量,促进患者社会功能康复,帮助患者回归社会。Objective To investigate the effect of case management on quality of life and social function of chronic schizophrenia patients. Methods To offer case management service for 150 chronic schizophrenia patients in Pengjiang and Jianghai district in Jiang Men city, and then evaluate their quality of life and social function by Positive and Negative Symptom Scale for schizophrenia （ PANSS ） , Insight and Treatment Attitudes Questionnaire（ITAQ） , Schizophrenia Quality of Life Scale（SQLS） , personal and social perform- anee scale（PSP） before the service and 24 months after the service. Results The PANSS and SQLS scores of patients after 24 months service are significantly lower in those of before the service, P 〈 0.05. The ITAQ and PSP scores of patients after 24 months service are significantly higher than those of before the service, P 〈 0.05. Conclusion Case management service can effectively control the disease, improve the quality of life and promote the rehabilitation of social function of patients.
