碳酸锂合并氨磺必利治疗双相障碍Ⅰ型抑郁发作的对照研究

摘　　要：目的对碳酸锂联合氨磺必利治疗双相障碍Ⅰ型抑郁发作的效果及不良反应进行研究。方法选取2013年3月-2014年3月在青岛市精神卫生中心住院的双相障碍Ⅰ型抑郁发作的患者70例,均符合《精神疾病诊断与统计手册(第4版)》(DSM-IV)诊断标准,采用随机数字表法将入组患者分为研究组和对照组,各35例。研究组给予碳酸锂联合氨磺必利治疗,对照组给予碳酸锂治疗,所有入组患者分别在治疗开始时及开始后第1、2、4、6周末应用汉密尔顿抑郁量表(HAMD-17)评定疗效,副反应量表(TESS)评定不良反应。结果研究组共完成33例,对照组共完成32例。两组在治疗开始时HAMD-17评分差异无统计学意义(P>0.05),HAMD-17评分总分减分率在治疗的第1、2、4、6周末升高,差异有统计学意义(P<0.05)。两组主要不良反应差异无统计学意义(P>0.05)。结论碳酸锂联合氨磺必利治疗双相障碍Ⅰ型抑郁发作与单用碳酸锂相比,起效快,未见明显的不良反应。Objective To investigate the clinical efficacy and safty of Lithium Carbonate combined with Amisulpride in the treatment of bipolar I depression. Methods 70 outpatients with bipolar I depression were chosen and taken open treatment for 6 weeks. All patients were randomly divided into two groups. The study group （35 cases） was given Lithium Carbonate combined with Amisulpride, and the control group （35 cases） was given Lithium Carbonate only. The efficacy and adverse reactions at baseline, end of 1 st, 2nd, 4th, and 6th week after the treatment were assessed by Hamilton Depression Scale （HAMD - 17 ） and Treatment Emergent Symptom Scale （TESS） respectively. Results There were 33 and 32 patients respectively in study group and control group finishing the study. At the baseline, the difference of HAMD- 17 scores between research group and control group was no significant （P 〉 0.05 ）. At the end of 1 st, 2nd, 4th, 6th week after treatment, the HAMD - 17 scores decreasing rate in the study group were significantly higher than the control group （ P 〈 0.05 ）. There were no significant differences of adverse reactions between the two groups （ P 〉 0.05 ）. Conclusion The treatment of Lithium Carbonate eombined with Amisulpride takes effect more quickly than Lithium Carbonate, the adverse reactions and the total efficiency are similar.
