低位直肠癌套入式吻合保肛术患者延续性康复需求与自我康复能力调查分析

摘　　要：目的了解低位直肠癌套入式吻合保肛术患者对延续性康复需求和自我护理能力情况,并探讨两者的相关性。方法采用自行设计的低位直肠癌套入式吻合保肛术患者延续性康复需求调查问卷和自我护理能力量表,对55例低位直肠癌套入式吻合保肛术患者进行调查分析。采用SPSS 17.0统计软件进行数据分析。延续康复需求和自我护理能力两者关系采用Pearson相关分析,P<0.05有统计学意义。结果本组患者延续性康复需求总分(68.85±4.21)分,自我护理能力总分(90.36±19.51)分,两者呈负相关,有统计学意义(r=-0.268,P<0.05)。自我护理能力总分与专科康复护理(r=-0.685,P<0.01)及应急救护总分(r=-0.196,P<0.01)呈负相关,与健康教育需求呈正相关(r=0.560,P<0.01)。自我护理技能与专科康复护理(r=-0.695,P<0.01)及需求总分(r=-0.395,P<0.01)呈负相关,健康知识水平与专科康复护理(r=-0.505,P<0.01)及健康教育需求(r=-0.708,P<0.01)呈负相关。结论患者自我护理能力水平越低,对延续性康复需求越高,提高自我护理技能,加强健康教育,可促进患者自我康复。Objective To investigate the extended-care needs and self-care agency in patients who underwent anus-preserving rectectomy by telescopic anastomosis for low rectal cancer , and to analyze their correlations. Methods Fifty-five patients who underwent anus-preserving rectectomy by telescopic anastomosis for low rectal cancer were investigated with the Extended-Care Needs Scale and the Exercise of Self-Care Agency Scale （ESCA）.Data analysis was performed by using SPSS 17.0 statistical software. Pearson correlation analysis was performed between extended-care needs and self-care agency.P〈0.05 was considered statistically significant .Results The total score of extended-care needs was （68.85 ±4.21） and that of self-care agency was （90.36 ±19.51）.There was a negative correlation between them （r=-0.268, P〈0.05）.The total score of self-care agency was negatively correlated with specific nursing （r=-0.685, P〈0.01） and emergency care （r=-0.196, P〈0.01）, and positively correlated with health education （r=0.560, P〈0.01）.Self-care skills were negatively correlated with specific nursing （r=-0.695, P〈0.01） and that of extended-care needs （r=-0.395, P〈0.01）.Level of health knowledge was negatively correlated with needs of specific nursing （r=-0.505, P〈0.01） and health education （r=-0.708, P〈0.01）. Conclusions Lower self-care agency patients have the more extended-care they need .To improve patients ’ self-care agency and strengthen their health education could promote patients ’ self-care ability.
