飞行人员消化性溃疡复发因素的随访研究

摘　　要：目的探讨飞行人员消化性溃疡复发的特点,为溃疡病的防治提供科学参考。方法对26例消化性溃疡飞行人员进行随访观察,分别在年龄、飞行时间、溃疡部位、飞行机种、飞行职务、幽门螺杆菌(Helicobacter pylori,Hp)感染这6个方面各分为两组(组间具有可比性),比较两组溃疡的复发率。结果 26例随访观察(26±3.5)个月,高质量愈合4例(15.4%),中质量愈合19例(73.1%),低质量愈合3例(11.5%)。26例中复发6例(23.1%),其中胃溃疡1例,十二指肠壶腹溃疡3例,复合溃疡2例。≤30岁与>30岁溃疡复发率差异无统计学意义(P>0.05);与短时间飞行相比较,长时间飞行溃疡病的复发率较高,差异有统计学意义(P<0.05);与其他机型比较,战斗机飞行员溃疡复发率较高,差异有统计学意义(P<0.05);飞行员与其他人员比较,溃疡复发率更高,差异有统计学意义(P<0.05);以十二指肠溃疡为主的溃疡组与以胃溃疡为主的溃疡组比较,复发率更高,差异有统计学意义(P<0.05);Hp阳性者与阴性者比较,阳性者溃疡病的复发率更高,差异有统计学意义(P<0.05)。结论飞行人员消化性溃疡复发与多种因素相关,主要包括长时间飞行、战斗机飞行员、溃疡主要在十二指肠壶腹部、Hp阳性等。重视飞行人员心理保健和根除Hp,是防治飞行人员消化性溃疡复发的重要举措。Objective To investigate the recurrence feature of aircrew＇s peptic ulcer,and to provid scientific evidence for prevention and treatment of peptic ulcer. Methods Twenty-six cases of newly diagnosed peptic ulcer aircrew were divided into two groups according to the following six aspects： age, flight time, position of ulcer, air plane species, duty of flying, and helicobacter pylori（Hp） infection rate. The recurrence rates of peptic ulcer in these six aspects were compared respectively. Results Twenty-six cases of newly diagnosed peptic ulcer aircrew were followed-up for（26±3.5）months, 4 cases were high-quality heal（15.4%）, 19 cases were mid-quality heal（73.1%）, 3 cases were low-quality heal（11.5%）. 6 cases recurred among 26 cases： 1 case was gastric ulcer,3 cases were duodenal ulcer, 2 cases were complex ulcer. There was not statistically significant difference in age between the group of older than 30-year-old and the group of younger than 30-year-old（P〉0.05）. While there were statistically significant difference in the part of flight time, position of ulcer, air plane species,duty of flying, Hp infection（all P〈0.05）. The recurrence rate of peptic ulcer in the group of long-time flying was higher than that in the group of short-time flying; the recurrence rate of peptic ulcer in the group of battleplane was higher than that in the group of other types of aircraft; the recurrence rate of peptic ulcer in the group of pilot was higher than that in the group of other personnel; the recurrence rate of peptic ulcer in the group of duodenal ulcer was higher than that in the group of gastric ulcer; the recurrence rate of peptic ulcer in the group of Hp positive was higger than that in the group of Hp negtive. Conclusions The recurrences of peptic ulcer in aircrew are concerned with multiple factors. The major factors include： flying for a long time, pilots of battle plane, duodenal ulcer, helicobacter pylori（Hp）infextation, and so on. Reconstruction of psychological health and
