横切口对白内障伴角膜高度散光患者视功能和立体视觉的影响

摘　　要：目的探讨横切口对白内障伴高度角膜散光患者视功能及立体视觉的影响。方法收集单眼白内障伴角膜散光大于2.0D患者40例,随机分为两组,A组20例(20眼),B组20例(20眼)。A组行11:00钟位,3.0mm透明角膜切口超声乳化白内障摘出联合后房型人工晶体植入术;B组首先完成同A组的手术操作,然后以最陡峭子午线为中心,做成对横切口。观察患者术后3个月裸眼视力、角膜散光、远立体视锐度。结果两组术后裸眼视力比较差异有统计学意义(P<0.01);角膜散光矢量A组J0及P术前与术后比较差异无统计学意义(P>0.05),B组J0及P术后与术前比较差异有显著统计学意义(均P<0.01),A组与B组术后3个月J0及P值比较差异有显著统计学意义(P<0.01);A组术后远立体视锐度在100″以上,B组在70″以下,B组术后远立体视锐度优于A组(P<0.01)。结论横切口可以提高白内障伴高度角膜散光患者的视功能及立体视觉。Objective To evaluate the effect of＂T＂incision on distant stereovision of cataract patients with high cornea astigmatism.Methods 40age-related cataract patients（40eyes）with high cornea astigmatism for surgery in our hospital were divided into two groups：A（20eyes）an B（20eyes）.The A groups received superior（11：00）clear corneal incision（CCI）.The B groups besides the superior（11：00）CCI,for surgery on the steepest meridian,an additional couple of＂T＂incisions were made to release the preexisting corneal astigmatism.The operation eyes were all implanted Rayner foldable posterior chamber intraocular lens.The UCVA,corneal astigmatism,distant stereovision were observed at 3months after operation.Results The UCVA in each group after surgery was all better than that of preoperation,difference being statistically significant（P〈0.01）.the difference between Group A and Group B was significant（P〈0.05）;In Group A,there were no significant difference on the corneal J0 and Pbefore and after operation（P〉0.05）,The corneal J0 and Pin group B after surgery were lower than that of preoperation.difference being statistically significant（P〈0.01）.difference between Group A and Group B（P〈0.01）at 3months postoperation was statistically significant（P〈0.01）;The stereopsis in each group after surgery was all better than that of preoperation,difference being statistically significant（P〈0.01）.the difference between Group A and Group B postoperation was significant（P〈0.05）.Conclusion the＂T＂incisions on the steepest corneal meridian can improve the patient′s visual function and distant stereovision.
