抗磷脂抗体与体外受精-胚胎移植结局的Meta分析

摘　　要：目的系统评价抗磷脂抗体(APA)与体外受精-胚胎移植(IVF-ET)结局之间的关系。方法计算机检索Cochrane Library、MEDLINE、EMbase、中国生物医学文献数据库、万方数据库等,收集符合纳入标准的研究,检索时限均为从建库至2013年3月,并追溯纳入研究的参考文献和手工检索相关会议资料。由两位研究者按照纳入与排除标准独立筛选文献、提取资料和评价质量后,采用RevMan 5.0软件进行Meta分析。结果共纳入1 160例研究对象。结果显示:抗磷脂抗体(APA)与IVF-ET后胚胎种植失败率、临床妊娠率、活产率、流产率之间未发现明显关联(P>0.05),合并OR值及95%CI分别为0.98,(0.78,1.24);1.01,(0.81,1.26);1.02,(0.80,1.29);1.13,(0.60,2.11)。结论根据现有的临床证据,APA与IVF-ET结局间无显著联系,对拟行IVF-ET的妇女是否常规行APA检测尚需进一步研究,是否对仅APA阳性而无相关临床症状的患者进行治疗有待商榷,对APA的作用机制需进行深入研究。Objective：To assess the association between antiphospholipid antibody and outcome of in vitro fertilization and embryo transfer（IVF-ET）.Methods：All references were retrieved from databases including Cochrane Library,MEDLINE,Embase,CBM,CNKI,VIP and Wanfang data.Data were collected from inception to March 2013.The references of included studies and relevant conference proceedings were also retrieved manually.Two reviewers independently screened the literature according to the inclusion and exclusion criteria,extracted the data,and assessed the quality.Then RevMan 5.0 software was used for Meta-analysis.Results：There were 1 160 cases retrieved in this study.The association was assessed between antiphospholipid antibody and outcome of IVF-ET.Their pooled OR and 95 ％ CI for embryo implantation failure rate,clinic pregnancy rate,live birth rate and abortion rate were 0.98 （0.78,1.24）,1.01 （0.81,1.26）,1.02（0.80,1.29）,1.13（0.60,2.11）.Conclusions：The results suggested that there is no statistically association between antiphospholipid antibody and outcome of IVF-ET.Antiphospholipid antibody should not be routinely detected in pregnancy women.The related mechanism still needs to be deeply carried out.
