卵巢癌中COX-2和PTEN的表达及其临床意义

摘　　要：目的探讨卵巢癌组织中环氧合酶2(COX-2)和PTEN蛋白表达及其临床意义。方法免疫组化染色检测卵巢癌(A组,96例)和正常卵巢组织(B组,40例)COX-2和PTEN蛋白表达。结果 A组COX-2蛋白表达水平(IOD值)高于B组(94.6±33.3vs.41.5±14.5)(P<0.01),而PTEN表达水平低于B组(38.4±12.6vs.91.2±31.3)(P<0.01)。与高分化、无淋巴结转移和临床Ⅰ、Ⅱ期的卵巢癌比较,中、低分化、有淋巴结转移和临床Ⅲ、Ⅳ期卵巢癌中COX-2表达升高,PTEN表达降低(P<0.05)。卵巢癌组织中COX-2和PTEN的表达呈显著负相关(r=-0.506,P<0.05)。COX-2阴性患者的3年累积生存率为38.6%,高于COX-2阳性患者的22.6%(P<0.05);PTEN阴性患者的3年累积生存率为21.5%,低于PTEN阳性患者的56.4%(P<0.05)。结论卵巢癌组织中COX-2和PTEN蛋白的异常表达参与了卵巢癌的发生、发展过程,有可能作为卵巢癌分级、预后评估的新指标。Objective To detect the expresssions and clinical significance of cyclooxygenase-2 （COX-2） and phosphatase and tensin bomologue（PTEN） in the patients with ovarian cancer. Methods The expressions of COX-2 and PTEN were detected by immunohistochemical analysis in ovarian cancers of 96 patients（group A） and normal ovarian tissues of 40 cases（group B）. Results The expression of COX-2 was higher in group A than that in group B（94.6±33.3 vs. 41.5±14. 5） （P〈0. 01） ,which of PTEN was lower in group A than that in group B（38. 4±12.6 vs. 91.2±31.3） （P〈0. 01）. Compared to the ovarian cancers with high differentiation, non-lymph node metastasis and clinical stages of I or Ⅱ , the expression of COX-2 was higher and that of PTEN was lower in the ovarian cancers with meddle or low differentiation,lymph node metastasis and clinical stages of Ⅲ or Ⅳ （P〈0. 05）. The COX-2 in ovarian cancer was negatively correlated with PTEN （r=--0. 506, P〈0. 05）. Three-year survival rate was lower in the cases with COX-2 positive than that in those with COX-2 negative（22.6% vs. 38. 6%）（P〈0. 05）, which was higher in the eases with PTEN positive than that in those with PTEN negative（56.4% vs. 21.50%）（P〈0. 05）. Conclusion The upregulation of COX-2 and downregulation of PTEN are involved in the occurrence and development of ovarian cancer, which can be used as the indicators to assess the grade and prognosis of ovarian tumor.
