摘 要:目的:观察牙周炎复合肥胖大鼠血清及牙龈组织中炎症因子的水平与胰岛素抵抗的关系。方法:取SPF级新生SD大鼠32只,随机分为正常对照组(C)、肥胖组(OB)、牙周炎组(CP)、牙周炎复合肥胖组(OB+CP)(n=8),通过皮下注射谷氨酸钠建立肥胖模型;丝线结扎联合局部接种牙周致病菌(Pg、Ha、Pi、Fn)建立牙周炎模型。采用ELISA法和免疫组化染色法分别检测大鼠血清及牙龈组织中的TNF-α、IL-1β含量,并与胰岛素抵抗指数(HOMA-IR)行相关性分析。结果:与正常对照组相比,各组血清中的TNF-α和IL-1β水平均明显升高(P<0.05),其中以OB+CP组升高幅度最大(P<0.05);HOMA-IR评分也以OB+CP组最高,肥胖大鼠血清中TNF-α及IL-1β水平与HOMA-IR均呈正相关(rTNF-α=0.854,rIL-1β=0.730)。各组牙龈组织中的TNF-α及IL-1β水平以CP组最高(P<0.05),其次为OB+CP组,其TNF-α及IL-1β水平低于CP组(P<0.05),但均高于C组和OB组(P<0.05);OB组牙龈组织中的TNF-α、IL-1β水平与正常对照组差异均无统计学意义(P>0.05)。结论:牙周炎可能通过促进血清中炎症因子的分泌而加剧肥胖机体的胰岛素抵抗。AIM:To investigate the expression of TNF-αand IL-1βin serum and gingival tissue in differ-ent rat models so as to analyze the relationshiop of periodontitis with insulin resistance.METHODS:32 SPF SD rats were randomly divided into control (C),obese (OB),chronic periodontitis (CP)and obese plus periodontitis (OB+CP)groups(n=8).Obese rat model was established by subcutaneous injection of sodium glutamate.Chronic peri-odontitis was induced by periodontal ligation and inoculation of periodontal pathogens.Serum and gingival TNF-αand IL-1βexpression was measured by ELISA and immunohistochemical staining respectively.The relationship of TNF-αand IL-1βexpression with insulin resistance index (HOMA-IR)was analyzed by statistical methods.RESULTS:OB+CP group showed the highest serum TNF-αand IL-1βlevels than other groups (P〈0.05 ),while OB and CP groups showed higher levels of TNF-αand IL-1βthan group C (P〈0,05 ).Furthermore,OB+CP group had the highest HOMA-IR score.Corralation analysis showed that serum TNF-αand IL-1βlevels were positively related to HOMA-IR (rTNF-α=0.854,rIL-1β=0.730).In gingival tissues,the expression of TNF-αand IL-1βwere in the following group order:CP〉OB+CP〉C and OB.Gingival TNF-αand IL-1βexpression in OB+CP group was lower than that in CP group but was higher than that in C and OB groups (P〈0.05).Gingival TNF-αand IL-1βex-pression in OB and C groups was not significantly different (P〉0.05).CONCLUSION:Periodontitis might enhance insulin resistance in obese rats through increasing the secretion of serum inflammatory factors.
