奥硝唑羧甲基壳聚糖配合基础治疗对牙周炎疗效的影响

摘　　要：目的:评价奥硝唑羧甲基壳聚糖溶胶注入牙周袋后对改善牙周临床指标的效果。方法:临床59例患者随机分成两组,试验组(n=28)采用奥硝唑羧甲基壳聚糖配合牙周基础治疗;对照组(n=31)只进行牙周基础治疗。分别记录基线(0 d)和治疗后7、14、21、28 d菌斑指数(PLI)、出血指数(BI)、探诊深度(PD)、附着丧失(AL)的改变;记数螺旋体数量(S%)、观察龈下菌斑中胰蛋白酶样酶(TLF)水平量OD值的变化。结果:基线时试验组和对照组各项临床指标无显著差异(P>0.05)。两组治疗前后各时间点比较,除PD、AL差异无显著意义外(P>0.05),其他各指标差异均P<0.05,且试验组在降低PLI、BI的程度、减少S%和OD值方面均明显优于对照组(P<0.05)。治疗后28 d时试验组与对照组相比PLI、PD、AL均无改变(P>0.05),而BI、S%和OD值差异显著(P<0.05),试验组临床疗效(89.3%)优于对照组(75.8%)(P<0.05)。结论:牙周袋内联合应用奥硝唑羧甲基壳聚糖溶胶比单纯牙周基础治疗效果更好。AIM：To examine the effect of ornidazole and carboxymethyl chitosan gel（OCCG）as an adjunct in the treatment of periodontal diseases.METHODS：A randomized,single blind and controlled clinical trial was per-formed.59 periodontitis patiens were randomly divided into periodontal therapy only group （control,n=31）and perio-dontal therapy plus OCCG group （n=28）.Plaque index （PLI）,bleeding index （BI）,probing depth （PD）,attache-ment loss （AL）,the number of spirochete（s）and trypsin-like proteinase in subgingival plaque were measured and recorded at baseline,7,14,21 d and 28 d after treatment.RESULTS：Similar parameters were found between the 2 groups at baseline（P〉0.05）.After treatment,both groups showed significant decrease in clinical parameters except PD and AL at all time points （P〈0.05）.The experiment group showed better effects in the decrease of PLI,BI,S%and OD value than control group （P〈0.05）.The effective rate of experiment and control group5 were 89.3% and 75.8% respectively （P〈0.05）.COCLUSION：Ornidazole and carboxymethyl chitosan gel as an adjunct are favora-ble in the treatment of periodontal diseases.
