腹腔镜器械在开腹卵巢癌肿瘤细胞减灭术中的应用

摘　　要：目的探讨腹腔镜器械（超声刀和血管闭合切割系统）在开腹卵巢癌肿瘤细胞减灭术中的应用价值。方法回顾性分析2010年1月～2013年6月45例开腹卵巢癌肿瘤细胞减灭术的临床资料，前30例使用传统开腹器械，后15例使用腹腔镜器械。比较2组的切口长度、手术时间、术中出血量、术后排气时间以及术后并发症的差异。结果与传统开腹器械相比，使用腹腔镜器械可缩小切口长度[（13．4±2．2）emVS．（20．2±2．7）cm，t=－8．391，P：0．000]，缩短手术时间[（160．7±30．1）min vs．（205．7±31．3）min，t=一4．610，P=0．000]，减少术中出血量[（286．7±76．7）mlVS．（403．3±80．9）ml，t=－4．639，P=0．000]，缩短住院时间[（10．3±2．2）dVS．（12．1±2．3）d），t：－2．523，P=0．015]。2组术后均无并发症发生。结论腹腔镜器械在开腹卵巢癌肿瘤细胞减灭术中有着良好的应用前景，值得推广。Objective To explore the utility of laparoscopic instruments （uhrasound scalpel and LigaSure vessel sealing system） in open eytoreductive surgery for ovarian cancer. Methods We retrospectively reviewed the clinic data of 45 patients who received open cytoreductive surgery for ovarian cancer from January 2010 to June 2013. Conventional surgical instruments were employed in the first 30 patients （ open instrument group）, while ]aparoscopie instruments were used in the following 15 patients （laparoscopic instrument group）. The incision length, operation time, intraoperative blood loss, postoperative recovery time of gastrointestinal function, hospital stay, and complications were compared between the two groups. Results The laparoscopic instrument group showed significantly shorter incision length [ （ 13.4 ±2.2） cm vs. （20.2 ± 2.7 ） cm, t = - 8. 391, P = 0. 000 ] , shorter operation time [（160.7±30.1） min vs. （205.7 ±31.3） min, t = -4.610, P=O. O00], less intraoperative blood loss [（286.7±76.7） ml vs. （403.3 ±80.9） ml, t= -4.639, P=0.0001, shorter hospital stay [（10.3 ±2.2） d vs. （12.1 ±2.3） d, t = -2. 523, P = 0. 015 ] than those in the open instrument group. No complication occurred in the two groups. Conclusion Application of laparoscopic instruments, including ultrasonic scalpel and LigaSure vessel sealing system, is promising in the open surgery for cytoreductive surgery of ovarian cancer.
