重性精神病患者家庭负担与家属焦虑抑郁反应的相关分析

摘　　要：目的:探讨重性精神病患者的家庭负担与家属的焦虑抑郁情绪之间的关系。方法:采用自编的一般家庭状况调查问卷、家庭负担会谈量表和焦虑自评量表、抑郁自评量表对265例重性精神病患者的一级亲属进行评定。结果:重性精神病患者家属焦虑自评量表的标准分、抑郁自评量表标准分远高于正常人群(t=50.47,P<0.001;t=11.41,P<0.001)。家庭负担的各个因子与抑郁严重度指数、焦虑自评量表的标准分均呈显著正相关(P<0.001)。在多因素分析中,对焦虑自评量表标准分影响的严重程度,依次是疾病家庭负担量表总分、病前家庭经济状况、家庭成员心理健康、病后家庭经济状况、病情稳定程度;对抑郁自评量表标准分影响的严重程度,依次是病后家庭经济状况、病前家庭经济状况、入过何种保险。结论:疾病的家庭负担,尤其是经济负担会严重影响家属的焦虑抑郁情绪。Objective：To explore relationship between family burden of severe psychotic patients and anxiety and depression emotion of their family members. Methods：265 cases of first- degree relatives of the severe psychotic patients were assessed by self-made general family situation questionnaire,family burden scale,self- rating anxiety scale,and self- rating depression scale. Results： The standard scores of self- rating anxiety scale and self- rating depression scale of the family members of the severe psychotic patients were found far higher than normal population（t = 50. 47,P〈0. 001; t = 11. 41,P〈0. 001）. Each factor of family burden was significantly correlated with the standard scores of self- rating depression scale and self- rating anxiety scale（P〈0. 001）. In the multivariate analysis,the factors affecting self- rating anxiety scale were total score of family disease burden scale,premorbid family economic situation,family members＇ mental health,family economic situation after disease,degree in stable condition orderly; while the factors affecting self- rating depression scale were family economic situation after disease,premorbid family economic situation,types of insurance orderly. Conclusions： Family burden of the disease,especially economic burden,would seriously affect anxiety and depression emotion of the family members.
