有晶状体眼后房型人工晶状体植入术后视觉对比敏感度观察

摘　　要：目的 观察有晶状体眼后房型人工晶状体植入术后视觉对比敏感度的变化.方法 前瞻性临床病例研究.对2010年3月至2012年6月在武警陕西省总队医院眼科就诊的高度近视患者29例共55只眼,实施有晶状体眼后房型人工晶状体植入手术.观察患者术前及术后1、4、12、24周视力和屈光度等变化,对术前与术后24周对比敏感度进行比较.结果 所有手术眼裸眼视力明显提高.术后24周裸眼视力≥1.0的眼数较术前最佳矫正视力≥1.0增加4只眼,并有34只眼（61.8％,34/55）裸眼视力高于术前最佳矫正视力.术后24周患者对比敏感度在明视环境下,6、18c/d空间频率段较术前明显提高,差异有统计学意义（P＜0.05）;在暗视和暗视眩光环境下,12、18c/d空间频率段与术前比较均明显提高,差异有统计学意义（P＜0.05）.结论 高度近视患者实施有晶状体眼后房型人工晶状体植入手术,能明显提高不同亮度环境下,中、高空间频率段的对比敏感度,改善视觉质量.Objective To evaluate the contrast sensitivity after implanting the posterior chamber phakic intraocular lens.Methods Twenty-nine patients （55 eyes） received posterior chamber phakic intraocular lens implantation.Visual acuity,refraction and the contrast sensitivity were examined before twenty-four weeks after the operation.Results After the surgery all of the patients had a higher uncorrected visual acuity （UCVA）.Twenty-four weeks after the operation there were 4 eyes （7.3%） with the UCVA≥ 1.0 more than the eyes with the best corrected visual acuity （BCVA） ≥ 1.0preoperatively.After surgery the UCVA of 34 eyes （61.8%） was better than the best corrected visual acuity （BCVA） before surgery.Twenty-four weeks after the surgery,in bright condition,the contrast sensitivity in 6 and 18 c/d were significantly improved （P 〈0.05）.In dark and glare conditions,the contrast sensitivity in 12 and 18 c/d were significantly improved （P 〈0.05）.Conclusions The implantation of posterior chamber phakic intraocular lens in high myopica can improve the contrast sensitivity in high c/d in any light condition,and also imprvoe the visual quality.
